✔️ Dosage & Administration of Imax 20

Oral (Capsules/Tablets):

• Erosive Esophagitis: 20–40 mg once daily for 4–8 weeks (extendable if unhealed)
• Maintenance of Healing: 20 mg once daily (up to 6 months)
• GERD Symptoms: 20 mg daily for 4 weeks (extendable if unresolved)
• H. pylori Eradication: 40 mg once daily + Amoxicillin 1000 mg BID + Clarithromycin 500 mg BID for 10 days
• Zollinger-Ellison Syndrome: 20–80 mg daily, adjusted individually
• Acid Dyspepsia: 20–40 mg daily for 2–4 weeks
• Duodenal Ulcer: 20 mg daily for 2–4 weeks
• Gastric Ulcer: 20–40 mg daily for 4–8 weeks

1. Children: Not established.
2. Elderly: No major differences observed, but individual sensitivity may vary.
3. Liver Impairment: In severe hepatic insufficiency, limit dose to 20 mg/day.
4. Kidney Impairment: No dose adjustment needed as <1% is excreted unchanged.

Capsules/Tablets: Swallow whole, 1 hour before meals.

✔️ Drug Interactions

• iMAX is metabolized mainly via CYP2C19 and CYP3A4.
• Minimal interaction with drugs metabolized by other CYP enzymes.
• Co-admin with diazepam may increase diazepam levels.
• Reduced absorption of drugs dependent on acidic pH (e.g., ketoconazole, iron salts, digoxin).
• Safe to use with oral contraceptives, phenytoin, quinidine, and antibiotics like amoxicillin and clarithromycin.

✔️ Contraindications

• Known hypersensitivity to esomeprazole or any capsule component.
• No difference in side effects seen between short-term and long-term use (up to 12 months).

✔️ Use During Pregnancy & Lactation

• Limited human data; animal studies show no teratogenic effects.
• Unknown if excreted in breast milk—discontinue breastfeeding if necessary.

✔️ Precautions

• Rule out gastric malignancy before starting iMAX.
• Can be taken with antacids.
• For patients unable to swallow capsules: contents can be sprinkled on soft applesauce and swallowed immediately (no chewing or storage).
