Indications of Carbocal 500 Tablet

250 mg or 500 mg tablet: This is used to treat or prevent calcium deficiency in people whose dietary measures are insufficient. Hypoparathyroidism, achlorhydria, chronic diarrhea, vitamin D insufficiency, steatorrhea, sprue, pregnancy and lactation, menopause, pancreatitis, renal failure, alkalosis, and hyperphosphataemia are all conditions that may be linked with calcium shortage. Calcium carbonate is increasingly being utilized to treat hyperphosphataemia in patients with chronic renal failure, as well as those undergoing continuous ambulatory peritoneal dialysis (CAPD) and haemodialysis.Many individuals are unable to tolerate enough dosages for full phosphate management, necessitating additional treatments such as dietary phosphate restriction or relatively low doses of aluminium hydroxide. Preparations containing calcium carbonate can give short-term relief for dyspeptic systems, but they are no longer indicated for long-term therapy of peptic ulcers.

1000 mg tablet: This is suggested for the treatment of hypertensive disorders as well as for the quick alleviation of acid indigestion, heartburn, sour stomach, and upset stomach.

Pharmacology of Carbocal 500 Tablet

When calcium carbonate interacts with stomach acid, a salt and water are formed. CaCO3 + 2HCl = CaCl2 + H2O + CO2 is the proposed chemical process for calcium carbonate.

Two grams of calcium carbonate will easily raise the pH of 100 mL of hydrochloric acid over 6. The activity of pepsin in gastric secretion decreases as stomach pH rises. Up to 30% of the calcium dose taken orally may be absorbed.

Dosage & Administration of Carbocal 500 Tablet

250 mg or 500 mg tablet: Calcium Carbonate is always used orally and when used as an antacid the recommended doses for adults are equivalent to 540-2000 mg Calcium Carbonate per day, doses for children being half of those for adults. As a dietary supplement, such as for the prevention of osteoporosis, 1250-3750 mg Calcium Carbonate (500-1500 mg calcium) daily is recommended in general, but again this will need to be tailored to the individual patient depending on any specific disease such as Calcium deficiency, malabsorption or parathyroid function. In pregnancy and lactation the recommended daily dose of calcium is 1200-1500 mg. In chronic renal failure the doses used vary from 2.5-9.0 gm Calcium Carbonate per day and need to be adjusted according to the individual patient. To maximize effective phosphate binding in this context the Calcium Carbonate should be given with meals.

1000 mg tablet: 2000-3000 mg tablet when symptoms occur; may be repeated hourly if needed or as directed by the physician.

Interaction of Carbocal 500 Tablet

If systemic hypercalcaemia develops, calcium carbonate may increase the cardiac effects of digoxin and other cardiac glycosides. When Calcium Carbonate is used as the major phosphate binder, it may interfere with the absorption of concomitantly given tetracycline formulations, and in chronic renal failure, vitamin D medication may need to be modified to avoid hypercalcaemia.

Contraindications

Hypercalcaemia and hyperparathyroidism

Hypercalciuria and nephrolithiasis

Zollinger-Ellison syndrome

Concomitant digoxin therapy (requires careful monitoring of serum calcium level)

When hypercalcaemia arises, stopping the medication typically suffices to restore normal blood calcium values. Calcium salts should be used with caution in individuals with sarcoidosis, renal or cardiac illness, or those on cardiac glycosides.

Side Effects of Carbocal 500 Tablet

Calcium carbonate used orally has the potential to irritate the GI tract. It may also result in constipation. Hypercalcaemia is seldom caused by calcium alone, although it can develop when substantial doses of calcium are administered to individuals with chronic renal failure.

Pregnancy & Lactation

Oral calcium supplementation or antacid treatment with calcium-containing medications has been widely utilized in pregnancy. Lactating women can also benefit from calcium carbonate.

Precautions & Warnings

This drug should be taken cautiously in patients with mild hypercalciuria
