- Prevention of atherothrombotic events in adults after an Acute Coronary Syndrome (ACS) with elevated cardiac biomarkers (e.g., Troponin or CK-MB).
- Administered with Aspirin alone or with Aspirin plus Clopidogrel or Ticlopidine.
Rivaroxaban is a highly selective direct Factor Xa inhibitor. By inhibiting Factor Xa, it disrupts both the intrinsic and extrinsic pathways of the coagulation cascade, preventing thrombin formation. It does not inhibit thrombin (activated Factor II) or affect platelets.

Active Ingredient: Rivaroxaban
Drug Class: Antithrombotic agents
Rafter 2.5 is prescribed for:
- Managing deep vein thrombosis (DVT) and pulmonary embolism (PE).
- Treating venous thromboembolism (VTE).
- Reducing the risk of stroke and systemic embolism in patients with nonvalvular atrial fibrillation.
- Preventing recurrence of DVT or PE.
- Lowering the risk of major cardiovascular events in patients with coronary artery disease (CAD).
- Reducing major thrombotic vascular events in patients with peripheral artery disease (PAD).
- Managing DVT in patients undergoing knee or hip replacement surgery.
- Treating VTE in acutely ill medical patients.
- Preventing thrombosis in children (older than 2 years) with congenital heart disease after the Fontan procedure.
Before using Rafter 2.5, inform your doctor if you:
- Have liver or kidney problems, bleeding disorders, or antiphospholipid syndrome.
- Are pregnant or breastfeeding, as the medication is not recommended in these situations.
- Are giving the medication to a child under 6 months who was born prematurely (<37 weeks), has had <10 days of oral feeding, or weighs <2.6 kg.
- Are elderly (65 years or older), as the medication should be used cautiously in this age group.
The most frequently reported side effects include:
- Bleeding (e.g., gums, nosebleeds, or unusual bruising).
- Pain or swelling in the limbs.
- Fever.
- Digestive issues: Stomach pain, indigestion, nausea, vomiting, diarrhea, or constipation.
- Low blood pressure: Dizziness or fainting upon standing.
- General discomfort: Weakness, tiredness, or headache.
- Skin reactions: Rash or itchy skin.
Contact your doctor if side effects persist or worsen.
- Not recommended for children younger than 6 months who meet specific criteria (premature birth, low weight, or inadequate oral feeding).
- Use caution in elderly patients, as they may be more susceptible to the side effects of the medication.

✔️ Dosage of Rivaroxaban 2.5 mg
- Recommended dose: 2.5 mg twice daily.
- Should be combined with 75-100 mg Aspirin daily, or 75-100 mg Aspirin along with either 75 mg Clopidogrel or a standard dose of Ticlopidine.
✔️ Common side effects include:
- Increased risk of bleeding.
- Spinal or epidural hematomas.
- Elevated risk of stroke following discontinuation in nonvalvular atrial fibrillation.
✔️ Overdose Effects
- Overdose may lead to hemorrhage. Absorption plateaus at doses above 50 mg.
- No specific antidote is available. Activated charcoal may reduce absorption in overdose cases.
- Plasma products can partially reverse laboratory anticoagulation parameters.
✔️ Interaction
- Avoid using with combined P-gp and CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, fluconazole, erythromycin, verapamil) as they increase Rivaroxaban exposure and its anticoagulant effects.
- Co-administration with combined P-gp and strong CYP3A4 inducers (e.g., rifampicin, phenytoin, carbamazepine) may reduce Rivaroxaban efficacy and should also be avoided.
- Concurrent use of other medications such as antiplatelet agents, heparin, fibrinolytics, or NSAIDs may heighten the risk of bleeding.
✔️ Contraindications
- Hypersensitivity to Rivaroxaban or its components.
- Active pathological bleeding.
✔️ Pregnancy & Lactation
- Pregnancy: Rivaroxaban is a Category C drug; its safety during pregnancy has not been established, and use is not recommended.
- Lactation: It is unknown if Rivaroxaban is excreted in breast milk. Its safety and efficacy in breastfeeding women are not established.
✔️ Precautions & Warnings
- Premature discontinuation without alternative anticoagulation increases the risk of thrombotic events.
- May cause severe bleeding in patients with certain conditions, including bleeding disorders, uncontrolled severe hypertension, or gastrointestinal diseases (e.g., inflammatory bowel disease, gastritis, or GERD).
- Increased risk of spinal or epidural hematoma in patients undergoing neuraxial anesthesia or spinal puncture; monitor for neurological impairment.
- Not recommended for patients with pulmonary embolism who are hemodynamically unstable or may require thrombolysis or embolectomy.
