✔️ Side Effects: Sliptin-M 1000 ER Tablet

Common side effects include diarrhea, upper respiratory tract infections, headache, hypoglycemia (in combination with insulin or sulfonylurea), and nasopharyngitis. Gastrointestinal effects such as nausea, vomiting, and abdominal discomfort are also common with metformin initiation.

✔️ Dosage & Administration: Sliptin-M 1000 ER Tablet

• Film-coated tablet: The dose should be individualized based on the patient's current regimen, effectiveness, and tolerability. The maximum daily dose should not exceed 100 mg of sitagliptin and 2000 mg of metformin.
• For patients not currently on metformin, the starting dose is 50 mg sitagliptin/500 mg metformin twice daily.
• For those already on metformin, the recommended starting dose is 50 mg sitagliptin/1000 mg metformin twice daily.
• Extended-release tablet: Take once daily with a meal, preferably in the evening. Gradually increase the dose to minimize gastrointestinal side effects.

✔️ Interactions: Sliptin-M 1000 ER Tablet

• Cationic drugs eliminated by renal secretion should be used with caution.
• Phenprocoumon: Metformin may decrease its anticoagulant effect, so INR should be monitored.
• Levothyroxine: May reduce metformin's hypoglycemic effect, requiring blood glucose monitoring and possible dose adjustment.

✔️ This tablet should not be used in patients with:

• Renal dysfunction (e.g., serum creatinine ≥1.5 mg/dL in males or ≥1.4 mg/dL in females).
• Acute or chronic metabolic acidosis, including diabetic ketoacidosis.
• A history of serious hypersensitivity reactions to sitagliptin or metformin.

✔️ Pregnancy & Lactation:

• Pregnancy Category B: No adequate studies have been conducted on pregnant women, so it should only be used if needed.
• It is not known if sitagliptin is excreted in human milk, so caution is advised for nursing women.

✔️ Precautions & Warnings:

• Lactic Acidosis: Metformin accumulation can cause lactic acidosis, especially in patients with renal impairment, sepsis, dehydration, or excessive alcohol intake. Discontinue the medication immediately if acidosis is suspected.
• Hematologic Monitoring: Annual monitoring is recommended.
• Pancreatitis: Discontinue if pancreatitis is suspected.
• Renal Function: Monitor renal function regularly.

Store below 25°C, in a dry place, away from light, and out of the reach of children. Do not use after the expiration date. To be dispensed only on a prescription from a registered physician.
