✔️ Pharmacology

Clobetasol Propionate is a super-potent topical corticosteroid possessing anti-inflammatory, antipruritic, and vasoconstrictive actions. It alleviates inflammation through several mechanisms that inhibit late-phase allergic responses. The drug reduces mast cell density, eosinophil activation and chemotaxis, and cytokine production, while also suppressing the metabolism of arachidonic acid.

✔️ Dosage and Administration

Route: Cutaneous (for external use only).

Cream & Ointment: Apply a thin film twice daily and gently rub in. For resistant or hyperkeratotic lesions, occlusion with polyethylene film overnight may enhance efficacy. Limit treatment to 2 consecutive weeks, not exceeding 50 g per week. In children, restrict use to 5 days, with weekly review.

Spray / Scalp Solution: Apply once or twice daily to the affected scalp area, not exceeding 50 ml per week. Massage gently with fingertips. Discontinue if no improvement is observed within a week or once healing occurs.

Shampoo: Apply to dry scalp once daily, leave for 15 minutes, then lather and rinse. Limit therapy to 4 weeks; do not exceed 50 g per week. Not recommended for patients under 18 years.

Form Selection:

Cream – ideal for moist or weeping lesions.

Ointment – suitable for dry, scaly, or lichenified lesions.

✔️ Side Effects

• Prolonged or extensive use may cause systemic absorption leading to hypercorticism. Local side effects include:
• Skin atrophy, striae, telangiectasia (especially under occlusion or in skin folds)
• Pigmentation changes, hypertrichosis, or pustular psoriasis upon withdrawal
• Hypersensitivity reactions and symptom aggravation in rare cases
• If any allergic reaction or worsening occurs, discontinue use immediately.

✔️ Drug Interactions

Medications that inhibit CYP3A4 (e.g., ritonavir, itraconazole) may reduce corticosteroid metabolism, resulting in increased systemic exposure and potential adverse effects.

✔️ Xderm should not be used in the following conditions:

• Rosacea, acne vulgaris, or perioral dermatitis
• Primary viral skin infections (e.g., herpes simplex, varicella)
• Fungal (e.g., candidiasis, tinea) or bacterial infections (e.g., impetigo)
• Perianal and genital pruritus
• Hypersensitivity to Clobetasol or any excipient
• Dermatoses in children under 1 year (including diaper dermatitis)

✔️ Pregnancy & Lactation

Limited human data are available. Animal studies indicate possible fetal developmental abnormalities following corticosteroid exposure. Use only if the potential maternal benefit outweighs fetal risk.
It is unknown whether the drug is excreted in human milk; therefore, caution is advised for nursing mothers.

✔️ Precautions & Warnings

• Avoid prolonged or continuous therapy, especially in children or on facial areas, to prevent adrenal suppression and skin atrophy.
• Do not use occlusive dressings on children.
• Prevent contact with eyes to avoid glaucoma or cataract.
• Use cautiously in psoriasis due to risks of rebound flare, tolerance, and pustular psoriasis.
• If infection develops, discontinue the corticosteroid and start appropriate antimicrobial therapy.
• Cleanse the skin before reapplying occlusive dressings to prevent bacterial growth.

Store below 30°C, away from light and moisture. Do not freeze.
