Elderly Individuals: Begin and maintain doses cautiously.

For Individuals with Hepatic Impairment: Avoid use due to the risk of lactic acidosis.

Child Dosage:

• For Type 2 Diabetes Mellitus in children above 10 years: Initiate with 500 mg once or twice daily or 850 mg once daily. The dose can be gradually raised to a maximum of 2000 mg daily in 2 or 3 divided doses, with intervals of at least 1 week.

Renal Impairment:

• Assess the estimated glomerular filtration rate (eGFR) before starting metformin.
• If eGFR is below 30 mL/min/1.73 m²: Metformin is contraindicated.
• If eGFR is between 30-45 mL/min/1.73 m²: Starting metformin is not recommended. Monitor eGFR at least annually, especially for those at risk.
• If eGFR drops below 45 mL/min/1.73 m² during metformin therapy, evaluate the benefits and risks.
• If eGFR falls below 30 mL/min/1.73 m² during metformin use, discontinue the medication.
