How does Feel 20mg Tablet work?

Feel 20mg Tablet functions by inhibiting specific enzymes (PDE5) in the body that restrict blood flow to the penis. This action relaxes the blood vessels supplying the penis, enhancing blood flow and sustaining an erection during sexual intercourse.

Furthermore, it relaxes the smooth muscles around the bladder and prostate gland, enhancing urine flow and alleviating the uncomfortable symptoms of Benign Prostatic Hyperplasia.

• Decreased hearing or sudden hearing loss

• Avoid certain foods like sugars, carbonated beverages, tea, citrus fruits, tomatoes, spicy foods, chocolate, and tea.
• Limit fluid intake to prevent frequent urination.
• Reduce alcohol and caffeine consumption as they can worsen symptoms.
• Maintain a healthy weight and engage in regular exercise.
• Quit smoking.
• Stay adequately hydrated with 6-8 glasses of fluids daily.
• Choose whole, unprocessed foods over processed ones.
• Include fruits, vegetables, and fiber-rich foods in your diet.
• Take Feel 20mg Tablet 30 minutes to 4 hours before sexual intercourse, with an hour before being ideal.
• Do not exceed one dose of Feel 20mg Tablet in a day.
• Always follow your doctor's prescribed dosage.

Feel 20mg Tablet belongs to the group of medicines known as phosphodiesterase type 5 (PDE 5) inhibitors. It is primarily prescribed for the treatment of erectile dysfunction (impotence) and benign prostatic hyperplasia (BPH). Additionally, it can be used to manage pulmonary arterial hypertension, a condition characterized by high blood pressure in the lungs.

Tadalafil is a phosphodiesterase type-5 inhibitor.

Dosage & Administration of Feel 20mg Tablet

Tadalafil can be taken with or without food, and it requires sexual stimulation to be effective. It can be taken 30 minutes to 12 hours before sexual activity, and its effects may last up to 24 hours.

For erectile dysfunction:

• Starting dose: 10 mg before anticipated sexual activity.
• Dose adjustment: Can be increased to 20 mg or decreased to 5 mg based on individual response.
• Maximum dosing frequency: Once per day for most patients.

For once-daily use:

• Starting dose: 2.5 mg taken at the same time every day.
• Dose adjustment: This can be increased to 5 mg based on individual response.

For benign prostatic hyperplasia:

• Dose: 5 mg once daily, regardless of sexual activity timing.
• Combination with finasteride: Tadalafil 5 mg plus finasteride 5 mg once daily for up to 26 weeks.

For pulmonary arterial hypertension:

• Dose: 40 mg once daily; avoid dividing the dose.
• Patients also taking ritonavir: May start with 20 mg once daily for over a week, with a potential increase to 40 mg based on tolerability.

Hepatic impairment (erectile dysfunction):

• Mild-to-moderate: Not to exceed 10 mg once daily.
• Severe: Not recommended.

Hepatic impairment (pulmonary arterial hypertension):

• Mild-to-moderate: Consider starting with 20 mg once daily.
• Severe: Avoid use.

Renal impairment (erectile dysfunction):

• Mild: No dosage adjustment needed.
• Moderate: Not to exceed 5 mg once daily initially, with a maximum dosage of 10 mg every 48 hours.
• Severe (CrCl <30 mL/min and on hemodialysis): Not to exceed 5 mg every 72 hours.

Renal impairment (pulmonary arterial hypertension):

• Mild-to-moderate: Start with 20 mg once daily, with a potential increase to 40 mg daily based on tolerability.
• Severe (CrCl <30 mL/min and on hemodialysis): Avoid use.

Interaction

• Increased risk of hypotension (low blood pressure) when used with beta-blockers.
• Increased risk of priapism (painful, prolonged erection) when used with other drugs for erectile dysfunction, such as alprostadil.
• Increased heart rate when used with theophylline.
• Decreased tadalafil serum concentration (lower levels of the medication in the bloodstream) when used with CYP 3A4 inducers like rifampicin, efavirenz, carbamazepine, nevirapine, barbiturates, phenobarbital, phenytoin, or rifabutin.
• Increased tadalafil serum concentration (higher levels of the medication in the bloodstream) when used with CYP3A4 inhibitors like azole antifungals, protease inhibitors, cimetidine, or macrolides.

Contraindications

Do not use with organic nitrates, nitric oxide donors, or in men with certain cardiac conditions. Avoid if recent heart attack or stroke, low blood pressure, unstable angina, heart failure, uncontrolled arrhythmias, or hypertension.

Pregnancy & Lactation

Feel 20mg Tablet is not suitable for use in women.

Precautions & Warnings

Feel 20mg Tablet should be used with caution in the following situations or conditions:

1. Anatomic deformation of the penis
2. Cardiovascular disease
3. Left ventricular outflow obstruction
4. Myocardial infarction within the past 90 days
5. Unstable angina
6. Angina occurring during sexual intercourse
7. NYHA class 2 or greater heart failure within the past 6 months
8. Uncontrolled arrhythmias
9. Hypotension
10. Uncontrolled hypertension
11. Cerebrovascular accident within the past 6 months
12. Bleeding disorders
13. Active peptic ulcer disease
14. Liver disease
15. Renal impairment
16. Conditions predisposing to priapism
17. Concomitant use of CYP3A4 inhibitors
