Indications

• This formulation is indicated for the short-term treatment of resistant eczemas.
• Neurodermatitis.
• In psoriasis, secondary bacterial or fungal infection is present, suspected, or likely (excluding extensive plaque psoriasis).
• Less potent steroids haven't worked well for other inflammatory disorders.

Pharmacology

Clobetasol Propionate has the potential to be an extremely potent corticosteroid. It is recommended for the treatment of extremely hot skin conditions such as dermatitis and psoriasis that have not responded to weaker corticosteroids. Neomycin Sulfate is an aminoglycoside antimicrobial that is used to treat diseases caused by microscopic organisms. Nystatin is an antifungal that kills parasites and yeasts by interacting with their cell layers. The common instrument of topical steroids such as Clobetasol is hazy. Clobetasol Propionate, on the other hand, is a highly active corticosteroid with anti-inflammatory properties. Clobetasol Propionate's main effect on the skin may be a nonspecific anti-inflammatory reaction, possibly due to vasoconstriction and a decrease in collagen union. Neomycin binds to the ribosomal 30s and 50s subunits of vulnerable microscopic organisms and inhibits protein aggregation. Neomycin also causes a misreading of the genetic coding of the mRNA template, resulting in the incorporation of incorrect amino acids into the developing polypeptide chain. Nystatin works by attaching to sterols in the cell membrane of the fungus, causing a change in membrane permeability that allows intracellular components to flow out.

Dosage & Administration

Adults: Apply sparingly to the affected area once or twice daily until improvement occurs. In very resistant lesions, especially where there is hyperkeratosis, the anti-inflammatory effect of this preparation can be enhanced (if necessary) by occluding the treatment area with polythene. Treatment should not be continued for more than 7 days without medical supervision. If a longer course is necessary, it is recommended that treatment should not be continued for more than 4 weeks without the patient's condition being reviewed.

Elderly: This preparation is suitable for use in the elderly. Caution should be exercised in cases where a decrease in renal function exists and significant systemic absorption of Neomycin Sulphate may occur.

Children: This preparation is suitable for use in children (2 years and over) at the same dose as adults. A possibility of increased absorption exists in very young children, thus this cream/ointment is not recommended for use in neonates and infants (younger than 2 years).

Interaction

Following extensive systemic absorption, neomycin sulfate can exacerbate and extend the respiratory depressive effects of neuromuscular blocking drugs. Systemic exposure to Neomycin Sulphate is expected to be modest if administered according to the instructions, and medication interactions are unlikely to be substantial. There have been no reports of dangerous interactions with Clobetasol Propionate or Nystatin.

Contraindications

Rosacea, acne vulgaris, and perioral dermatitis, as well as primary cutaneous viral infection (e.g., Herpes simplex, chicken pox) and hypersensitivity to the preparation, are all contraindications.

Side Effects

As with other topical corticosteroids, delayed utilization of expansive sum or treatment of broad ranges can result in adequate systemic assimilation to deliver the highlights of hypercortisolism. The impact is more likely to happen in newborn children and children and in the event that occlusive dressings are utilized. Delayed and serious treatment with profoundly dynamic corticosteroid arrangements may cause neighborhood atrophic changes within the skin such as diminishing, striae, and dilatation of the shallow blood vessels, especially when occlusive dressings are utilized, or when skin folds are included. Pregnancy & Lactation

There is little evidence to support the use of topically administered Neomycin during pregnancy and breastfeeding. However, because neomycin in the maternal blood can pass the placenta and pose a potential risk of fetal harm, the preparation should not be used during pregnancy or lactation. Clobetasol Propionate's safety in lactating women has not been demonstrated.

Precautions & Warnings

Adrenal suppression can occur even without blockage, therefore long-term continuous topical medication should be avoided wherever possible, especially in infants and children. If the preparation is applied to the eyelids, it must be carefully monitored to ensure that it does not enter the eye, as this could cause glaucoma. If the drug gets into the eye, the afflicted eye should be rinsed out completely with plenty of water.
