Isosorbide mononitrate is the main active breakdown product of isosorbide dinitrate and is responsible for most of its effects.
It relaxes blood vessel walls (vascular smooth muscle), which causes widening of veins and arteries — especially veins.
It also widens the coronary arteries, improving blood supply to the heart muscle.
The exact importance of each of these effects in relieving angina is not fully known.
Usually 20 mg twice daily — first dose in the morning, second dose 7 hours later. A lower starting dose (10 mg) may be used if needed. Maintenance dose may range from 20–120 mg daily (1–6 tablets), as directed by your doctor.
Phosphodiesterase inhibitors (e.g., sildenafil, tadalafil, vardenafil) – may cause dangerous drops in blood pressure.
Riociguat (a guanylate cyclase stimulator).
Other vasodilators or alcohol may increase blood pressure–lowering effects.
Use with calcium channel blockers may cause significant drops in blood pressure; dose adjustments may be needed.

Pregnancy: Category B — animal studies show no harm to the fetus, but there are no well-controlled studies in pregnant women. Use only if needed.
Breastfeeding: Unknown if it passes into breast milk. Use with caution.
