Gardasil IM Injection

Gardasil is a vaccine for the prevention of diseases caused by specific types of Human Papillomavirus (HPV).

For girls and women (ages 9-26):

• Prevention of cervical, vulvar, vaginal, and anal cancers caused by HPV types 16 and 18.
• Prevention of genital warts caused by HPV types 6 and 11.
• Prevention of precancerous lesions:
  • Cervical Intraepithelial Neoplasia (CIN) grades 1, 2, and 3.
  • Vulvar Intraepithelial Neoplasia (VIN) grades 2 and 3.
  • Vaginal Intraepithelial Neoplasia (VaIN) grades 2 and 3.
  • Anal Intraepithelial Neoplasia (AIN) grades 1, 2, and 3.

For boys and men (ages 9-45):

• Prevention of anal, oropharyngeal, and other head and neck cancers caused by HPV types 16, 18, 31, 33, 45, 52, and 58.
• Prevention of genital warts caused by HPV types 6 and 11.
• Prevention of precancerous lesions:
  • Anal Intraepithelial Neoplasia (AIN) grades 1, 2, and 3.

Pharmacology

The Gardasil vaccine is based on HPV-like virus particles (L1 VLPs) that stimulate the body’s immune response. The vaccine induces humoral immune responses against HPV types, providing protection against diseases caused by these virus strains. The exact mechanism of protection is not fully understood.

Dosage

For ages 9–14:

• 2-dose schedule: Administered at 0 and 6-12 months.
• If the second dose is given earlier than 5 months after the first, a third dose should be given at least 4 months after the second dose.
• 3-dose schedule: Administered at 0, 2, and 6 months.

For ages 15-45:

• 3-dose schedule: Administered at 0, 2, and 6 months.

Administration

• Intramuscular use only.
• Shake well before use to ensure the vaccine remains properly suspended.
• Administer in the deltoid (upper arm) or the anterolateral thigh.
• Observe for 15 minutes post-vaccination to monitor for syncope.

Interaction

• Immunosuppressive therapies (e.g., corticosteroids, cytotoxic drugs) may reduce immune response to the vaccine.

Contraindications

• Severe allergic reaction (hypersensitivity) to the vaccine or yeast (a component of the vaccine).

Side Effects

Common adverse reactions (≥10% incidence):

• Girls and women (16-26): Injection-site pain (89.9%), swelling (40%), erythema (34%), headache (14.6%).
• Girls (9-15): Injection-site pain (89.3%), swelling (47.8%), erythema (34.1%), headache (11.4%).
• Women (27-45): Injection-site pain (82.8%), swelling (23.3%), erythema (16.9%), headache (13.6%).
• Boys and men (16-26): Injection-site pain (63.4%), swelling (20.2%), erythema (20.7%).
• Boys (9-15): Injection-site pain (71.5%), swelling (26.9%), erythema (24.9%).

Pregnancy & Lactation

• Pregnancy: Category B. No evidence of harm to the fetus in animal studies, but safety during pregnancy has not been well-established. Use only if necessary.
• Lactation: Unknown if excreted in human milk. Caution is advised when administered to nursing women.

Precautions & Warnings

• Syncope: Observe patients for 15 minutes post-vaccination, as fainting can occur.
• Pediatric use: Safety and efficacy not established for children under 9 years.
• Geriatric use: Not evaluated in individuals over 65 years of age.

Overdose Effects

• Overdoses are generally well-tolerated, with adverse effects similar to those reported at recommended doses.

Storage Conditions

• Refrigerate at 2-8°C.
• Do not freeze.
• Protect from light. Administer as soon as possible after removal from refrigeration.
