How does Clopixol Depot Injection work?

Zuclopenthixol is a typical antipsychotic medicine that works by restoring the balance of certain chemical substances (neurotransmitters) in the brain. It exerts its actions by antagonizing the effects of dopamine by blocking the D1 and D2 dopamine receptors which improves the symptoms of schizophrenia, making it easier to function in everyday life.

Side Effects of Clopixol Depot Injection

• Drowsiness, blurred vision, tachycardia, nausea, dizziness, headache, excitement, postural hypotension, hyperprolactinemia, sexual dysfunction, ECG changes, hyperthermia, extrapyramidal symptoms, urinary frequency or incontinence, tardive dyskinesia.
• Potentially fatal: Neuroleptic malignant syndrome, blood dyscrasias.

Pharmacology

Zuclopenthixol, a thioxanthene derivative, binds strongly to dopamine D1 and D2 receptors. It also has a high affinity for α1-adrenergic and 5-HT2 receptors, with weaker activity at histamine H1 receptors, and an even lower affinity for muscarinic cholinergic and α2-adrenergic receptors.

Dosage & Administration of Clopixol Depot

Zuclopenthixol Depot: Requires close supervision after initiation to avoid over-medication or insufficient symptom control. Oral antipsychotics may be needed initially. The maintenance dose is typically 150-300 mg IM every 2-4 weeks, adjusted as needed.

Renal Dose:

• Dose reduction may be needed for renal impairment.

The injection is given into the muscle of the buttock or thigh. Can be taken with or without food.

Clopixol Depot may interact with:

• Tricyclic antidepressants
• Guanethidine and similar blood pressure medications
• Barbiturates and similar medications
• Levodopa and Parkinson’s medications
• Metoclopramide (gastrointestinal disorders)
• Other antipsychotics (e.g., thioridazine)
• Heartbeat-altering medications (e.g., quinidine, amiodarone, sotalol, erythromycin, moxifloxacin, lithium)

Contraindications

• Acute intoxication from alcohol, barbiturates, or opiates
• CNS depression, comatose states, suspected or confirmed subcortical brain damage, or circulatory collapse
• Known hypersensitivity to thioxanthenes, zuclopenthixol, or any component in the formulation

Pregnancy & Lactation

• The safety of zuclopenthixol during pregnancy has not been established.
• Zuclopenthixol is excreted in human milk; its safety during lactation is not established.

Precautions & Warnings

Use caution if you:

• Have used Clopixol Depot or other schizophrenia treatments before and had issues
• Are taking other medications
• Are pregnant, planning to become pregnant, or breastfeeding
• Consume alcohol regularly
• Have liver problems, Parkinson’s disease, seizures, dementia, glaucoma, kidney problems, risk factors for stroke, hypokalemia, hypomagnesemia, cardiovascular history, family history of blood clots, or are being treated for cancer
• Are using other antipsychotic medications
