Clarigen 5 is not recommended for use in patients with kidney failure. It should be used with caution in patients with other kidney problems.

Generally well-tolerated; infrequent reports include headache, dry mouth, fatigue, and rash.
Adults & children ≥12 years: 5 mg once daily
Hepatic impairment: No dosage change required for isolated liver issues
Renal impairment:
Mild (CrCl 50–80 ml/min): 2.5 mg once daily
Moderate (CrCl 30–50 ml/min): 2.5 mg every other day
Severe (CrCl 10–30 ml/min): 2.5 mg twice weekly (every 3–4 days)
End-stage renal disease (CrCl <10 ml/min) or patients on dialysis: Avoid use
For pediatric chronic idiopathic urticaria (6 months–17 years), dosing is extrapolated from adult efficacy data.
Allergy to levocetirizine, cetirizine, or hydroxyzine
Severe renal impairment (CrCl <10 ml/min)
Category B: No adequate human studies; use only if necessary
Excreted in breast milk; not recommended for breastfeeding mothers
