Indications

Lidocaine Jelly is indicated for-

• urethral surface anesthesia in both sexes
• Painful urethritis topical therapy
• Endotracheal intubation with anesthetic lubrication
• sufficient anesthesia during endoscopy.

Pharmacology

The main sodium-specific ion channels in the membrane of neuronal cells that lidocaine inhibits are voltage-gated sodium channels. When sodium influx is disturbed and signal conduction is impeded, an action potential cannot form. The cytoplasmic (inner) region is thought to contain the sodium channel's receptor site. Rapidly firing neurons experience neural blockage more quickly than slower-firing neurons because lidocaine binds to sodium channels more readily in an active state. The term for this circumstance is a state-dependent blockage.

Dosage & Administration

Urethral anesthesia:

• Adult male urethral surface anesthesia: 400 mg is needed to provide sufficient analgesia in males.
• Female adult urethra surface anesthesia: Inject 100–200 mg in small doses to completely fill the urethra. The urological procedure should wait a few minutes in order to achieve a sufficient level of anesthesia.
• 100 mg of lubricant should be applied to the tube's surface just before insertion to facilitate endotracheal intubation. It is important to take precautions to prevent getting the product inside the tube's lumen.
  
  Endoscopy:
• For adequate analgesia, an installation of 200–400 mg is advised, and a small amount should be applied to the instrument for lubrication. Children, elderly patients, and patients who are physically incapacitated should receive doses appropriate to their age and health.
• Note: After being sterilized for five minutes in boiling water, the applicator is cooled and attached to the tube. As needed, the jelly is slowly injected. After each use, the applicator should be removed, and the tube should always be kept tightly closed.

Interactions

Because the toxic effects of lidocaine and antiarrhythmic medications like tocainide are cumulative, they should be taken with caution in these patients.

Contraindications

It is not recommended for patients who have a history of hypersensitivity to amide-type local anesthetics.

Pregnancy & Lactation

Although there is no or insufficient evidence of the drug's safety in human pregnancy, it has been widely used for many years with no obvious adverse effects. If drug therapy is required during pregnancy, this medicine can be used if no other option is available. Lidocaine does enter the mother's milk, but in such little amounts that the child is rarely affected at therapeutic dose levels.

Precautions & Warnings

• Mucous membranes and wound sites typically have high absorption rates, particularly in the bronchial tree. Lidocaine Jelly should be used cautiously in patients with traumatized mucosa and/or infections in the area of the proposed application.
• Xylogel 2% is prescribed to treat eczema, burns, bites, rashes, and scrapes that cause itching and pain.
• Use a clean fingertip to apply it over the affected area three to four times per day, or as your doctor instructs.
• Except as directed by your doctor, avoid applying it to broken or infected skin, face, eyes, or eyelids.
• Without a doctor's prescription, avoid covering the area being treated with airtight dressings like bandages as this could increase the risk of side effects.
• Keep your distance from your mouth, nose, and eyes. If accidental contact occurs, thoroughly rinse with water.
• In patients with epilepsy, compromised cardiac health, bradycardia, impaired hepatic function, and severe shock, lidocaine, like other local anesthetics, should be used with caution if the dosage or site of administration is likely to produce high blood levels.
• Topical anesthetics for the oropharynx may make it difficult to swallow, which raises the possibility of aspiration.
• After 2-4 weeks of treatment, if your skin condition has not improved, see a doctor.

Storage Conditions
