Indications:

Vida Plus 850mg Tablet combines Metformin and Vildagliptin, offering potent treatment for type 2 diabetes (non-insulin-dependent diabetes mellitus) in adults. It's an ideal option when diet, exercise, or other medications like insulin or sulphonylureas (e.g., glimepiride, glipizide) alone can't adequately control diabetes.

Pharmacology

Vildagliptin primarily operates by inhibiting DPP4 (dipeptidyl peptidase 4), an enzyme responsible for breaking down incretin hormones such as GLP1 (glucagon-like peptide 1) and GIP (glucose-dependent insulin-promoting polypeptide). When vildagliptin is administered, it rapidly and completely inhibits DPP4 activity, resulting in elevated levels of the incretin hormones GLP1 and GIP, both in a fasting state and after meals. This increase in endogenous incretin hormone levels stimulates insulin secretion in pancreatic β cells while reducing glucagon secretion in α cells. The higher levels of incretin hormones also shift the insulin/glucagon ratio during hyperglycemia, leading to reduced fasting and post-meal hepatic glucose production and, consequently, a decrease in blood sugar levels.

On the other hand, metformin hydrochloride is an oral hypoglycemic agent belonging to the biguanide class. It is employed in the treatment of type 2 diabetes and works differently from sulfonylureas, avoiding the risk of hypoglycemia. Metformin operates by lowering both baseline and post-meal blood sugar levels through several mechanisms. It diminishes liver glucose production, reduces intestinal glucose absorption, and enhances insulin sensitivity by promoting peripheral glucose uptake and utilization.

Dosage & Administration of Vida Plus 850mg Tablet

Adult Dose: For individuals with Type 2 diabetes:

  • Based on their current Metformin dose, the Combination of Vildagliptin & Metformin can be initiated at either 50 mg/500 mg or 50 mg/850 mg, taken twice daily - one tablet in the morning and another in the evening.
  • The recommended maximum daily dose is 100 mg of Vildagliptin in combination with 2000 mg of Metformin HCl.
  • Patients who were previously taking Vildagliptin and Metformin as separate tablets may switch to a Combination of Vildagliptin and metformin with the same doses of each component.

Hepatic Impairment: This combination is not advised for patients with hepatic impairment, including those with pre-treatment ALT or AST levels greater than three times the upper limit of normal.

Child Dose: This combination is not recommended for patients under 18 years of age.

Renal Dose: For individuals with renal impairment:

  • This combination should not be used in patients with renal failure or renal dysfunction, indicated by serum creatinine levels exceeding 1.5 mg/dL in males or 1.4 mg/dL in females.

General Instructions: Take Vida Plus 850mg Tablet as prescribed by your physician. Consume the medication with or immediately after a meal, swallow the tablet whole, and do not crush or chew it. The appropriate dose and duration will be determined by your doctor based on your age and health condition. Do not discontinue the Vida Plus 850mg Tablet without informing your doctor.

Interaction

Before starting the Vida Plus 850mg Tablet, it's crucial to inform your doctor if you are currently taking any of the following medications:

  1. Glucocorticoids, such as dexamethasone and prednisolone, are used to treat inflammation.
  2. Beta-2 agonists like albuterol and terbutaline, are used to manage respiratory disorders.
  3. Diuretics, including chlorthalidone and hydrochlorothiazide, increase urine production.
  4. Pain and inflammation medications like ibuprofen and celecoxib.
  5. Blood pressure medications like angiotensin-converting enzyme (ACE) inhibitors (e.g., enalapril, lisinopril) and angiotensin II receptor antagonists (e.g., losartan, telmisartan).
  6. Medications affecting the nervous system and thyroid.
  7. Other diabetes medications like glimepiride and glipizide.
  8. Ranolazine is used to manage chest pain.
  9. Dolutegravir is prescribed for HIV infection.
  10. Vandetanib is utilized for medullary thyroid cancer.
  11. Cimetidine is employed to treat heartburn and peptic ulcers.

Since it is not clear whether vildagliptin and/or metformin hydrochloride will be excreted from human milk, this combination should not be used in breastfeeding women.

Precautions & Warnings

Vida Plus 850mg Tablet is not suitable for patients with type 1 diabetes or diabetic ketoacidosis. Some individuals with diabetes who take this medication may develop a rare but serious condition called lactic acidosis, characterized by an excess buildup of lactic acid in the bloodstream. Lactic acidosis can harm the functioning of the liver and kidneys, which are responsible for removing excess lactic acid from the blood.

Before initiating the Vida Plus 850mg Tablet, inform your doctor if you have a history of pancreatitis (inflammation of the pancreas), kidney disease, or low vitamin B12 levels. When Vida Plus 850mg Tablet is used in conjunction with insulin, it can significantly lower blood sugar levels, potentially leading to a life-threatening condition known as hypoglycemia. In such cases, your doctor may adjust the dosage by reducing the insulin or Vida Plus 850mg Tablet dose.

Long-term use of Vida Plus 850mg Tablet may lower thyroid-stimulating hormone (TSH) levels, so it's advisable to have an annual TSH check-up. In rare instances, a serious skin reaction called bullous pemphigoid may occur, necessitating immediate medical attention.

If you are scheduled to undergo a diagnostic test involving the injection of dye or X-ray contrast agents, inform your doctor. It may be necessary to discontinue the use of Vida Plus 850mg Tablets temporarily before undergoing an X-ray procedure. This precaution is important to ensure your safety during the diagnostic test.

Storage Conditions

Store Vida Plus 850mg Tablet in a cool and dry place, protected from direct sunlight, moisture, and heat. Keep it away from children and pets. Avoid using expired medicines. Discard unused medicine properly. Avoid flushing it into a toilet or throwing it into the drain.
