✔️ Rosuvastatin is a potent HMG-CoA reductase inhibitor, which works by:

• Increasing hepatic LDL receptor expression, enhancing LDL uptake, and breakdown.
• Suppressing hepatic VLDL production, thereby reducing VLDL and LDL particle concentrations.

✔️ Side Effects: Rtv 10

Generally well tolerated. Commonly reported effects include:

• Headache
• Muscle pain (myalgia)
• Constipation
• Weakness (asthenia)
• Abdominal pain
• Nausea

✔️ Dosage & Administration: Rtv 10

• Adults with high cardiovascular risk or very high cholesterol: 40 mg once daily.
• HoFH: Start at 20 mg/day.
• Pediatric patients (10–17 years, HeFH): 5–20 mg/day; max 20 mg.
• Asian patients: Start with 5 mg/day.
• With cyclosporine: Limit to 5 mg/day.
• With lopinavir/ritonavir or atazanavir/ritonavir: Max 10 mg/day.
• With gemfibrozil: Avoid if possible; if used, max 10 mg/day.
• Severe renal impairment (not on dialysis): Start at 5 mg/day, do not exceed 10 mg/day.

✔️ Drug Interactions:

• Cyclosporine: Increases RTV levels — max dose: 5 mg/day.
• Gemfibrozil: Avoid combination; if needed, limit RTV to 10 mg/day.
• Lopinavir/ritonavir or atazanavir/ritonavir: Limit RTV to 10 mg/day.
• Warfarin (Coumarin anticoagulants): May increase INR — monitor closely.
• Other lipid-lowering agents: Fibrates and niacin increase myopathy risk.

✔️ Contraindications:

• Known allergy to rosuvastatin or any component
• Active liver disease or persistent, unexplained liver enzyme elevations
• Pregnancy and lactation
• Women of childbearing potential not using contraception

✔️ Pregnancy & Lactation:

• Contraindicated in pregnancy and nursing.
• Women of childbearing potential should use effective contraception.
• Discontinue immediately if pregnancy occurs.

✔️ Precautions & Warnings:

• Use cautiously in elderly patients (>65 years), those with hypothyroidism, or renal impairment.
• Discontinue if creatine kinase is markedly elevated or myopathy is suspected.
• Temporarily stop during acute serious illness (e.g., sepsis, trauma, major surgery).
• Monitor liver enzymes before treatment, after 12 weeks of starting or dose adjustment, then periodically.
