✔️ Pharmacology
Iron:
- Essential for hemoglobin synthesis & oxidative processes
- Absorbed mainly in the duodenum & upper jejunum
- Ferrous form is absorbed 3× more readily than ferric
- Daily requirements:
- Adult male: ~1 mg
- Menstruating female: ~1.4 mg
- Last 2 trimesters of pregnancy: ~5–6 mg
Folic Acid:
- Required for normal cell division & erythropoiesis
- Deficiency → megaloblastic anemia
Timed release design:
- Prevents the release of iron in the stomach → fewer gastric side effects
- Releases iron at the absorption site → maximum bioavailability
✔️ Overdose Management
Symptoms: Epigastric pain, nausea, vomiting, haematemesis, circulatory collapse
Severe cases: encephalopathy, acute hepatic necrosis, acute renal failure (after a latent period)
Treatment:
- Gastric lavage
- 5 g of desferrioxamine in the stomach
- Monitor serum iron
- Severe cases → IV desferrioxamine + supportive care
Timed-release formulation may delay excessive absorption, allowing more time for intervention
✔️ Dosage & Administration
1 capsule daily during pregnancy & lactation
Higher doses may be needed in some patients (e.g., dietary insufficiency)
✔️ Drug Interactions
- Iron + tetracyclines → reduced absorption of both
- Iron absorption ↓ with magnesium trisilicate–containing antacids
Note: Tetracycline is contraindicated during pregnancy
✔️ Contraindications
Patients on repeated blood transfusions
Anemias not due to iron deficiency (unless iron deficiency is also present)
✔️ Precautions & Warnings
Avoid concurrent antacids/tetracyclines (reduced absorption)
Use with caution in patients prone to GI discomfort
