Treatment of Chemotherapy-induced nausea and vomiting

✔️ How does Aproxi 125 work?

Aproxi 125 contains Aprepitant, an anti-emetic (a drug used to treat nausea and vomiting) that works by blocking the action of a natural substance in the brain called neurokinin that causes nausea and vomiting. 

✔️ Side Effects of Aproxi 125

• Headache
• Constipation
• Loss of appetite
• Tiredness
• Indigestion
• Hiccups

These side effects are usually mild and go away over time. However, inform your doctor if they persist or worsen.

✔️ Quick Suggestions:

• Avoid eating over-sweet food and include more salty food, especially if you have been vomiting.
• Increase the intake of fluids. Drink a small amount of water after small intervals to stay hydrated.
• Avoid taking greasy or oily foods, as these foods trigger nausea and vomiting.

✔️ Aproxi is used for the prevention of:

• Postoperative nausea and vomiting (PONV)
• Chemotherapy-induced nausea and vomiting (CINV)

✔️ Postoperative Nausea & Vomiting (PONV)

• 40 mg taken orally within 3 hours before anesthesia induction.

✔️ Chemotherapy-Induced Nausea & Vomiting (CINV)

For Highly Emetogenic Chemotherapy:

• Day 1: Aprepitant 125 mg, Dexamethasone 12 mg, Ondansetron 24 mg (30 minutes before chemotherapy).
• Day 2 & 3: Aprepitant 80 mg, Dexamethasone 8 mg.
• Day 4: Dexamethasone 8 mg only.

For Moderately Emetogenic Chemotherapy:

• Day 1: Aprepitant 125 mg, Dexamethasone 12 mg, Ondansetron 8 mg (one tablet 30 minutes before chemotherapy, another 8 mg dose after 8 hours).
• Day 2 & 3: Aprepitant 80 mg, Ondansetron 8 mg twice daily.

Aprepitant should be taken 1 hour before chemotherapy on Day 1 and in the morning on Days 2 and 3.

✔️ Aproxi affects CYP3A4 and CYP2C9 enzymes, so caution is needed when used with:

• CYP3A4 inhibitors: Ketoconazole, Itraconazole, Ritonavir, Clarithromycin, etc.
• CYP3A4 inducers: Rifampin, Carbamazepine, etc.
• CYP2C9 substrates: Warfarin, Phenytoin, Tolbutamide, etc.

Hormonal contraceptives may be less effective during and up to 28 days after the last dose. Backup contraception is recommended.

✔️ Contraindications

• Hypersensitivity to Aprepitant or any of its components.
• Should not be used with: Pimozide, Terfenadine, Astemizole, or Cisapride.

✔️ Overdose Effects

• No specific antidote.
• Possible symptoms: Drowsiness, headache.
• Hemodialysis cannot remove Aproxi from the body. Supportive care is recommended.

✔️ Renal impairment:

• No dose adjustment is required.

✔️ Hepatic impairment:

No dose adjustment for mild to moderate impairment. No data for severe cases.

✔️ Pregnancy & Lactation

• Pregnancy Category B – Use only if necessary.
• Unknown if excreted in breast milk; consider stopping the drug or breastfeeding based on necessity.

✔️ Storage Conditions

• Store below 30°C, away from light & moisture.
• Keep out of reach of children.
