Untreated high blood pressure can have serious consequences, including:

• Stroke: High blood pressure can damage blood vessels in the brain, increasing the risk of a stroke. A stroke occurs when blood flow to a part of the brain is blocked or reduced, leading to brain damage.
• Heart Attack: The increased workload on the heart due to high blood pressure can lead to the development of coronary artery disease. This condition narrows the arteries that supply blood to the heart muscle, potentially causing a heart attack if a blockage occurs.

Pharmacology

As a result, the angiotensin II synthesis pathway has no bearing on its activity. When compared to the AT2 receptor, telmisartan's affinity for the AT1 receptor is >3000-fold higher. Telmisartan has no effect on the reaction to bradykinin because it does not inhibit ACE (kininase II). Other hormone receptors and ion channels known to play a role in cardiovascular regulation are not bound by or blocked by telmisartan.

Dosage & Administration of Telcardis 80mg Tablet

Telmisartan tablets are typically administered individually, with the usual starting dose being 40 mg once daily. The blood pressure response is dose-related within a range of 20 to 80 mg, with the maximum reduction in blood pressure usually achieved after 4 weeks, and the majority of the antihypertensive effect becoming evident after 2 weeks. If additional blood pressure control is needed beyond 80 mg of Telmisartan, a combination therapy may be considered.

Elderly patients and individuals with renal impairment, including those undergoing hemodialysis, typically do not require an initial dosage adjustment. However, it's crucial to closely monitor blood pressure in dialysis patients as orthostatic hypotension can develop. In some cases, other antihypertensive medications may be prescribed in conjunction with Telmisartan tablets.

Telmisartan tablets can be taken with or without food. However, for patients under 75 years of age or those with hepatic impairment, initiating therapy with telmisartan is not recommended.

Interaction

• Lithium: Telmisartan can potentially raise serum lithium levels, increasing the risk of lithium toxicity. Monitoring lithium levels and adjusting the dosage as needed is crucial when using both medications concurrently.
• Warfarin: Telmisartan may reduce the plasma levels of warfarin, potentially affecting its anticoagulant effect. Regular monitoring of INR (International Normalized Ratio) is recommended when Telmisartan and warfarin are used together.
• Potassium-Sparing Diuretics, Potassium Supplements, or Salt Substitutes: Telmisartan may increase the risk of hyperkalemia (elevated potassium levels) when used alongside potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium.
• NSAIDs (Non-Steroidal Anti-Inflammatory Drugs): Telmisartan's antihypertensive effect may be antagonized when used with NSAIDs, potentially leading to reduced blood pressure control and an increased risk of renal impairment. Monitoring blood pressure is advisable in such cases.
• Aliskiren in Patients with Diabetes and Renal Impairment: The concomitant use of Telmisartan and aliskiren in patients with diabetes and renal impairment (GFR <60 mL/min) may increase the risk of nephrotoxicity, hyperkalemia, and hypotension.
