Description:

Tofacent 5mg Tablet

  • Used to treat inflammatory diseases such as rheumatoid arthritis, psoriatic arthritis, ulcerative colitis, and ankylosing spondylitis.
  • Also used to treat polyarticular juvenile idiopathic arthritis and juvenile psoriatic arthritis in children.
  • Can be used alone or in combination with other drugs, such as methotrexate.
  • Not recommended for use in patients with severe liver diseases, bloodstream infections, or active tuberculosis.
  • Should be used with caution in patients with various conditions, including infections, chronic lung disease, tuberculosis, hepatitis B or C, liver, kidney, or heart diseases, cancer, skin cancer risk, smoking habits, lactose intolerance, inflammation of the large intestine, stomach or intestine ulcers, blood pressure, high cholesterol, at risk of developing a blood clot, planning to get vaccinated, or have undergone major surgery.
  • Not to be used by pregnant or breastfeeding women.
  • Caution in patients over the age of 65.
  • Safety and effectiveness in children under the age of two are not yet established.

How does Tofacent 5mg Tablet work?

Tofacent 5mg Tablet works by blocking the action of the enzyme, Janus kinase. This decreases inflammation and joint damage in rheumatoid arthritis.

Side Effects of Tofacent 5mg Tablet
  • The most common adverse reactions include upper respiratory tract infection, nasopharyngitis, diarrhea, and headache.
  • Reported in ≥5% of patients treated with Tofacitinib for ulcerative colitis: nasopharyngitis, elevated cholesterol levels, headache, upper respiratory tract infection, increased blood creatine phosphokinase, rash, diarrhea, and herpes zoster.

Indications of Tofacent 5mg Tablet

Tofacitinib 

  • For adult patients with moderately to severely active rheumatoid arthritis who have had an insufficient response or intolerance to methotrexate.
  • Can be used alone or in combination with other nonbiologic disease-modifying antirheumatic medicines like methotrexate.
  • Also indicated for adult patients with active psoriatic arthritis who have had an unsatisfactory response or sensitivity to methotrexate or other disease-modifying antirheumatic medications.
  • Used to treat ulcerative colitis in adults with moderately to highly active ulcerative colitis (UC).
Pharmacology
  • Tofacitinib is a Janus kinase (JAK) inhibitor that prevents the body from responding to cytokine signals.
  • Suppresses the phosphorylation and activation of STATs by inhibiting JAKs.
  • Blocks the JAK-STAT pathway, reducing the inflammatory response.
Dosage & Administration of Tofacent 5mg Tablet
  • Rheumatoid Arthritis: Tofacitinib 5 mg twice daily or Tofacitinib 11 mg once daily. Recommended dosage in patients with moderate and severe renal impairment or moderate hepatic impairment is Tofacitinib 5 mg once daily.
  • Psoriatic Arthritis: Tofacitinib 5 mg twice daily or Tofacitinib 11 mg once daily. Recommended dosage in patients with moderate and severe renal impairment or moderate hepatic impairment is Tofacitinib 5 mg once daily.
  • Ulcerative Colitis: Tofacitinib 10 mg twice daily for at least 8 weeks; then 5 or 10 mg twice daily. Discontinue after 16 weeks of 10 mg twice daily if adequate therapeutic benefit is not achieved. Use the lowest effective dose to maintain response.
Interaction
  • Inhibitors: Cytochrome P450 3A4 (CYP3A4) inhibitors of high potency (e.g., Ketoconazole) may require adjustment of Tofacitinib dosage.
  • Inducers: Inducers of CYP (e.g., Rifampin) may result in a lower clinical response to Tofacitinib.

  • Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus.
  • Not known whether Tofacitinib is excreted in human milk. Caution is advised during lactation.

  • Increased risk of gastrointestinal perforations.
