Indications

Pregabalin is recommended for the treatment of post-herpetic neuralgia and neuropathic pain brought on by diabetic peripheral neuropathy. Additionally, it is approved as an adjunctive treatment for patients aged one month and older experiencing partial-onset seizures. It can be used to treat fibromyalgia and neuropathic pain brought on by spinal cord damage.

Pharmacology

Gamma-aminobutyric acid, an inhibitory neurotransmitter, is the structural basis of the drug pregabalin (GABA). It doesn't directly bind to benzodiazepine, GABAA, or GABAB receptors. It does not inhibit dopamine, serotonin, or noradrenaline reuptake and is inactive at dopamine and serotonin receptors. In the tissues of the central nervous system, prelica@ binds to the alpha 2 -delta site (an auxiliary subunit of voltage-gated calcium channels) with a high affinity. Prelica@'s oral bioavailability is 90% or higher and dose-independent. In subjects with normal renal function, it is primarily excreted from the body through the kidneys as an unchanged drug with a mean elimination half-life of 6.3 hours.

Dosage & Administration

• The maximum dosage of Pregabalin that is advised for treating neuropathic pain brought on by diabetic peripheral neuropathy is 100 mg administered three times daily (a total of 300 mg per day) for patients with a creatinine clearance of at least 60 mL/min. Based on effectiveness and tolerability, the dosage should be started at 50 mg three times per day (150 mg/day) and increased to 300 mg/day within a week.
• Pregabalin is prescribed for post-herpetic neuralgia in patients with creatinine clearance of at least 60 mL/min at a dosage of 75 to 150 mg twice daily, or 50 to 100 mg three times daily (150 to 300 mg/day). The recommended dosage should be 75 mg twice daily or 50 mg three times daily (150 mg/day), with a possible increase to 300 mg/day after one week depending on effectiveness and tolerability.
• Adult patients receiving adjunctive therapy for partial-onset seizures: Generally speaking, patients should begin treatment with a daily dosage of no more than 150 mg (75 mg two times a day, or 50 mg three times a day). Depending on the response and tolerance of each patient, the dose may be increased up to 600 mg per day.
• Pregabalin is recommended for fibromyalgia at a dose of 300 to 450 mg per day. Based on effectiveness and tolerability, the dosage should be increased from 75 mg twice daily (150 mg/day) to 150 mg twice daily (300 mg/day) within a week.
• The recommended dosage range for neuropathic pain brought on by a spinal cord injury is 150 to 600 mg per day.
• The recommended starting dose is 75 mg twice daily (150 mg per day). Within a week, the dose may be increased to 150 mg twice daily (300 mg/day), depending on effectiveness and tolerance.
• You can take pregabalin pills with or without food.

Interaction

Pregabalin has no notable interactions with other antiepileptic medicines or oral contraceptives. Pregabalin has the potential to enhance the effects of ethanol and lorazepam.

Contraindications

Pregabalin is not recommended for individuals who have a history of hypersensitivity to Pregabalin or any of its components.

Side Effects

Common: Dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, and abnormal thinking.

Rare: Muscle weakness, muscle pain, breathing problems, diarrhea, etc.

Pregnancy: May cause fetal harm

Lactation: Breastfeeding is not recommended

Precautions & Warnings

Pregabalin withdrawal without tapering can result in insomnia, motion sickness, headaches, and diarrhea. Therefore, rather than stopping suddenly, it should be tapered gradually over a minimum of 1 week. Pregabalin treatment may cause an increase in creatinine kinase. If myopathy is identified or suspected, or if the level of creatinine kinase is noticeably elevated, it should be stopped right away.
