Indication

Oxicam 20mg Tablet is an analgesic that is used to relieve pain and inflammation in musculoskeletal conditions such as rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, tendonitis, etc. It can also be used to relieve acute painful episodes associated with acute gout, menstrual cramps, or post-operative pain. 

Pharmacology

Tenoxicam is a non-steroidal anti-inflammatory drug (NSAID) with anti-inflammatory, analgesic, and antipyretic properties. It also inhibits platelet aggregation. Tenoxicam inhibits prostaglandin biosynthesis. In-vitro tests of leukocyte peroxidase suggest that tenoxicam may act as a scavenger for active oxygen at the site of inflammation. Tenoxicam is a potent in-vitro inhibitor of human metalloproteinases (stromelysin and collagenase), which induce cartilage breakdown. These pharmacological effects explain, at least in part, the therapeutic benefit of tenoxicam in the treatment of challenging inflammatory and degenerative disorders of the musculoskeletal system. Tenoxicam showed no mutagenic, carcinogenic, or teratogenic effects in animals. As with other prostaglandin inhibitors, renal and gastrointestinal effects, increased frequency of dystocia, and delayed parturition were observed in animal safety studies.

Dosage & Administration:

Adult Dose:

• Oral administration for acute musculoskeletal disorders, osteoarthritis, and rheumatoid arthritis: 20 mg as a single dose, taken at the same time each day. Maximum: 40 mg/day (short-term use). Treatment duration: Up to 7 days for acute musculoskeletal disorders and up to 14 days for severe cases. In elderly patients, use the lowest effective dose for the shortest possible duration.

Administration:

• Should be taken with food. Take with or immediately after meals.

Interaction

• Increased risk of adverse effects (particularly GI) with salicylates and other NSAIDs: Concurrent use of tenoxicam with salicylates or other non-steroidal anti-inflammatory drugs (NSAIDs) may increase the risk of adverse effects, especially gastrointestinal (GI) effects.
• May enhance the anticoagulant effect of warfarin and other anticoagulants: Tenoxicam may enhance the anticoagulant (blood-thinning) effect of medications like warfarin, potentially increasing the risk of bleeding.
• May reduce the effect of antihypertensive drugs: Tenoxicam may reduce the effectiveness of drugs used to treat high blood pressure (antihypertensive drugs).
• Increased risk of nephrotoxicity with ciclosporin: Concurrent use of tenoxicam with ciclosporin may increase the risk of kidney toxicity.
• Increased risk of convulsions with quinolones: Tenoxicam may increase the risk of convulsions (seizures) when used concurrently with quinolone antibiotics.
• May decrease the elimination of lithium: Tenoxicam may interfere with the elimination of lithium from the body, potentially leading to increased levels and toxicity.
• May interfere with the natriuretic action of diuretics: Concurrent use of tenoxicam with diuretics may interfere with the natriuretic (salt-excreting) action of diuretic medications.
• May enhance the toxicity of methotrexate: Tenoxicam may enhance the toxicity of methotrexate, a medication used to treat various conditions, including rheumatoid arthritis.
• May reduce the effects of mifepristone: Tenoxicam may reduce the effectiveness of mifepristone, a medication used for medical abortion.
• Increased risk of GI bleeding with corticosteroids: Concurrent use of tenoxicam with corticosteroids may increase the risk of gastrointestinal (GI) bleeding.

Contraindications

Tenoxicam is contraindicated in individuals with:

1. Active or history of recurrent gastrointestinal (GI) bleeding or peptic ulcer.
2. Hypersensitivity to non-steroidal anti-inflammatory drugs (NSAIDs).
3. Severe heart failure.
4. Hemorrhagic diathesis (a tendency to bleed excessively).
5. Asthma.
6. Pregnancy during the third trimester.

Pregnancy & Lactation

Precautions & Warnings

Tenoxicam should be used with caution in patients with the following conditions or characteristics:

• History of active bronchial asthma.
• Uncontrolled hypertension (HTN).
• Congestive heart failure (CHF).
• Established ischemic heart disease.
• Peripheral arterial disease.
• Cerebrovascular disease.
• Risk factors for cardiovascular disease (e.g., hypertension, hyperlipidemia, diabetes mellitus, smoking).
• History of gastrointestinal (GI) disease.
• Fluid retention and edema.
• Patients who will undergo major surgery (e.g., joint replacement).
• Elderly patients.
• Renal impairment.
• Hepatic impairment.

Monitoring parameters should include regular assessments of renal, hepatic, and cardiac functions to ensure the safety of the patient during tenoxicam treatment.
