  • Treatment of obsessive-compulsive clutter.
Pharmacology

Escitalopram is an orally administered selective serotonin reuptake inhibitor (SSRI) used as an antidepressant medication. It is the S-enantiomer of citalopram, which is a racemic mixture of both S and R enantiomers. Escitalopram is known for its high selectivity and potency in inhibiting the reuptake of serotonin (5-HT), a neurotransmitter involved in regulating mood and emotions.

The S-enantiomer of citalopram, which is escitalopram, is at least 100 times more potent in inhibiting the reuptake of 5-HT compared to the R-enantiomer. This selectivity is considered beneficial as it allows for a more targeted and effective action on serotonin reuptake, leading to improved therapeutic effects in the treatment of depression and anxiety disorders.

Unlike other antidepressants or psychotropic medications, escitalopram has a very low affinity for other receptors in the brain, including alpha- and beta-adrenergic, dopamine, histamine, muscarinic, and benzodiazepine receptors. This selective action on serotonin reuptake is believed to contribute to its favorable side effect profile and reduced risk of adverse effects related to interactions with other receptors.

Overall, escitalopram's specificity for serotonin reuptake inhibition and its limited binding to other receptors make it a well-tolerated and effective antidepressant with a lower incidence of unwanted side effects compared to some other antidepressant medications.

Dosage & Administration of Esita 5mg Tablet

Esita 5mg Tablet is prescribed for the treatment of anxiety, depression, and obsessive-compulsive disorder in adults. 

  • The recommended Adult dose for these conditions is 10 mg once daily, which can be increased to a maximum of 20 mg once daily if needed. For panic disorder with or without agoraphobia, the initial adult dose is 5 mg once daily, which can be increased to 10 mg once daily after a week. The maximum daily dose for panic disorder is 20 mg.
  • For Elderly patients, the recommended dose is half of the adult dose. In patients with mild to moderate hepatic impairment, the initial dose is 5 mg daily, which can be increased to 10 mg daily after 2 weeks if needed. Patients with severe hepatic impairment may require more careful dose titration.
  • For children with major depressive disorder, the safety and efficacy of Esita 5mg Tablet have not been established in children below 12 years of age. For children above 12 years, the recommended dose is 10 mg once daily, which may be increased after at least 3 weeks if necessary, but the maximum daily dose should not exceed 20 mg.

Interaction

Increased Risk of Bleeding: Escitalopram may increase the risk of bleeding when used with other medications such as aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), or drugs that affect coagulation.

  • CYP2C19 and CYP3A4 Inducers: Certain drugs that induce the activity of the enzymes CYP2C19 and CYP3A4 can reduce the serum levels of escitalopram, potentially reducing its effectiveness. Examples of such drugs include carbamazepine, rifampin, phenytoin, nafcillin, and nevirapine.
  • CYP2C19 and CYP3A4 Inhibitors: Conversely, drugs that inhibit the activity of CYP2C19 and CYP3A4 can increase the serum levels of escitalopram, potentially leading to increased side effects or toxicity. Examples of such drugs include fluconazole, fluvoxamine, omeprazole, and azole antifungals, among others.
  • Interaction with Desipramine and Metoprolol: Escitalopram may increase the serum levels of desipramine and metoprolol, which are other medications used to treat various conditions. This can lead to potential side effects or altered drug effects.
  • Increased Risk of Serotonin Syndrome: Escitalopram should not be used with certain medications like linezolid or sibutramine, as it may increase the risk of serotonin syndrome, a potentially serious condition caused by excess serotonin in the body.
  • Interaction with Alcohol: Escitalopram may enhance the sedative effects of alcohol, leading to increased drowsiness or impairment.
  • Potentially Fatal Interactions: Concomitant use of escitalopram with monoamine oxidase inhibitors (MAOIs) or moclobemide can lead to serious or fatal reactions. Escitalopram should not be started until at least 2 weeks after stopping MAOIs or vice versa.
Contraindications

Escitalopram is contraindicated in patients with known touchiness to Escitalopram or Citalopram or any of the inert fixings of the sedate item. Concomitant utilization of escitalopram in patients taking monoamine oxidase/pimozide is contraindicated.

Pregnancy & Lactation

There are no sufficient and well-controlled studies on pregnant women to determine the safety of escitalopram during pregnancy. Therefore, its use during pregnancy should only be considered if the potential benefit to the mother justifies the potential risk to the fetus.

Escitalopram is excreted in human breast milk. Limited data from women taking 10-20 mg of escitalopram showed that exclusively breastfed infants receive approximately 3.9% of the maternal weight-adjusted dose of escitalopram and 1.7% of the maternal weight-adjusted dose of desmethyl citalopram. Caution should be exercised when administering escitalopram to a nursing woman, and breastfeeding infants should be observed for adverse reactions.

  • History of bleeding disorders: Patients with a history of bleeding disorders, especially gastrointestinal bleeding, should use SSRIs with caution.
  • Hepatic and renal impairment: Patients with impaired liver or kidney function should be monitored closely while taking SSRIs, as the medication's metabolism and elimination may be affected.
