Indication of Clopid 75mg Tablet

Clopid 75 is an antiplatelet medication, also known as a blood thinner, that aids in the prevention of harmful blood clots in your blood vessels. This reduces your chances of having a heart attack or stroke. It is a popular heart-protection medication. People at high risk of heart disease are given Clopid 75. This includes people who have peripheral vascular disease (blood circulation problems caused by blood vessel narrowing), a recent heart attack or stroke, irregular heartbeats, and those who have had certain heart procedures such as stenting.

Pharmacology

Clopidogrel is a prescription drug. It inhibits platelet activation and aggregation by binding irreversibly to platelet P2Y12 ADP receptors. A single oral dose can inhibit platelet aggregation dosage-dependently 2 hours later. Repeated doses of 75 mg per day reduce ADP-induced platelet aggregation on the first day, and inhibition achieves a stable state between Days 3 and 7.

Dosage & Administration of Clopid 75mg Tablet

• For the prophylaxis of thromboembolic disorders, recent myocardial infarction (MI), stroke, or established peripheral arterial disease, as well as coronary artery disease, the usual adult dose is 75 mg once daily.
• For acute coronary syndrome, the dose of Clopidogrel depends on the type of MI. For ST-elevation MI, the recommended dose is 75 mg once daily in combination with aspirin. Patients under 75 years of age may receive a loading dose of 300 mg followed by 75 mg for up to 12 months, while those over 75 years of age should not receive a loading dose and instead take 75 mg for 14 days up to 12 months (if no bleeding occurs). In all cases, the medication should be administered in combination with aspirin, with or without thrombolytics.
• For unstable angina and non-ST-elevation MI, the recommended dose is a 300 mg loading dose followed by 75 mg once daily for up to 12 months in combination with aspirin.
• Patients with hepatic impairment should use caution when taking Clopidogrel, as experience with the medication is limited.
• For patients with renal impairment, dose adjustment is not necessary.

Interaction

• NSAIDs, warfarin, selective serotonin, and serotonin-norepinephrine reuptake inhibitors (SSRIs, SNRIs): Increase the chance of bleeding
• Inhibitors of CYP2C19 (omeprazole or esomeprazole): Avoid taking omeprazole or esomeprazole at the same time.
• Clopidogrel (CYP2C8 substrates): Avoid using Clopid concurrently with Repaglinide since it raises plasma concentrations of Repaglinide.

Contraindications

If you have any of the following conditions, Clopid is not advised: hypersensitivity to any product component or the medication substance. Active pathological bleeding, such as a cerebral hemorrhage or peptic ulcer.

Pregnancy & Lactation

Precautions & Warnings

A CYP2C19 poor metabolizer and concurrent use of these medications may reduce Clopidogrel's antiplatelet activity.
• The risk of bleeding may increase because it prevents platelet aggregation over the course of the platelet's lifetime (7–10 days). Within 4 hours of the loading dose or 2 hours of the maintenance dose, platelet transfusions may have less success in reestablishing hemostasis.
• The risk of cardiovascular events increases with the discontinuation of clopidogrel. Five days before elective surgery with a high risk of bleeding, stop taking it. Once hemostasis has been achieved, clopidogrel should be restarted.
• There have been reports of TTP, which calls for immediate medical attention, including plasmapheresis (plasma exchange).
• Patients who have previously experienced hypersensitivity to other thienopyridines and are taking clopidogrel have experienced hypersensitivity, including rash, angioedema, or hematologic response.
