Description:

Viglimet 50/850mg Tablet is a combination medication that includes Metformin and Vildagliptin, belonging to the group of antidiabetic agents. It is used to treat type 2 diabetes in adult patients whose diabetes cannot be adequately controlled by diet, exercise alone, or other diabetes medications like insulin or sulphonylureas.

Type 2 diabetes is a chronic condition where the body either doesn't produce enough insulin or the produced insulin does not work effectively. Common symptoms of type 2 diabetes include excessive urination, thirst, hunger, fatigue, and blurred vision.

Metformin in Viglimet 50/850mg Tablet works by reducing glucose production in the liver, decreasing intestinal glucose absorption, and enhancing insulin sensitivity to improve peripheral glucose uptake and utilization. Vildagliptin, on the other hand, stimulates the pancreas to produce more insulin and less glucagon, leading to lower blood glucose levels.

During the treatment with Viglimet 50/850mg Tablet, it is essential to follow a regular exercise and diet plan as advised by the doctor to achieve better results.

Before taking Viglimet 50/850mg Tablet, inform your doctor if you have any liver, kidney, or heart problems. The use of Viglimet 50/850mg Tablet during pregnancy should only be done if advised by the doctor, and it is not recommended for breastfeeding women. This medication is not suitable for children and adolescents under 18 years of age, and caution should be exercised when using it in elderly patients (65 years and above).

How does Viglimet 50/850mg Tablet work?

Viglimet 50/850mg Tablet is a combination medication that helps in the management of type 2 diabetes. It contains two active ingredients, Vildagliptin (50 mg) and Metformin (850 mg).

Vildagliptin works by stimulating the pancreas to produce more insulin, which helps in lowering blood sugar levels. It also reduces the amount of glucagon, a hormone that raises blood sugar levels. This combination of actions helps control blood sugar levels after meals.

Metformin, on the other hand, works by reducing the production of glucose in the liver and improving the body's sensitivity to insulin. This helps the body utilize insulin more effectively, which further helps in lowering blood sugar levels.

By working together, Vildagliptin and Metformin play a vital role in controlling blood sugar levels and preventing serious complications of diabetes, such as retinopathy (eyesight loss), nephropathy (kidney damage), neuropathy (nerve damage), diabetic foot ulcers, and delayed wound healing.

Side Effects of Viglimet 50/850mg Tablet
  • Headache
  • Nausea
  • Vomiting
  • Diarrhea
  • Stomach ache
  • Trembling of hands and feet
Quick Suggestions:
  • Combine the medication with regular exercise and a healthy diet to help lower blood sugar levels.
  • Take the medication with food to reduce the chance of experiencing an upset stomach.
  • Viglimet 50/850mg Tablet has a lower likelihood of causing weight gain and low blood sugar compared to some other diabetes medicines.
  • If you are due to have surgery under a general anesthetic, inform your doctor about your diabetes treatment.
  • Plan your meals by filling half your plate with starchy vegetables, a quarter with proteins, and a quarter with whole grains.
  • Eat at regular intervals and avoid long gaps between meals or snacks.
  • Monitor your blood sugar levels regularly, especially when you experience fluctuations.
  • Engage in at least 150 minutes of moderate-intensity physical activity or 75 minutes of high-intensity exercise weekly.
  • If you need to lose weight, do so gradually to achieve a healthy body mass index (18.5 to 24.9).
  • Opt for whole grain foods and increase the intake of fruits, vegetables, and other fiber-enriched foods while limiting saturated fats found in unhealthy snacks.
  • Manage stress using techniques like mindfulness, as stress can impact blood sugar levels.
  • Choose low-fat dairy products to support a balanced diet.
  • Keep your blood pressure as close to normal (140/90) as possible to reduce the risk of cardiovascular diseases in diabetes patients.
  • If you experience symptoms such as nausea, vomiting, Kussmaul breathing (labored and deep breathing), and generalized weakness, contact your doctor immediately, as these could be signs of lactic acidosis, a rare but serious metabolic complication associated with the medication.
  • Inform your doctor if you experience symptoms such as abdominal pain, fatigue, loss of appetite, darkened urine, or yellowing of the eyes or skin (jaundice), as they may be related to kidney or liver function.
Indication of Viglimet 50/850mg Tablet

Type 2 Diabetes is a condition that causes abnormal fluctuations in blood sugar levels. In type 2 diabetes, the insulin produced by your body cannot be used properly by the cells to convert glucose into energy. Viglimet 50/850mg Tablet effectively reduces blood sugar levels in this condition.

Pharmacology

Vildagliptin primarily works by blocking the action of DPP-4 (Dipeptidyl peptidase-4), an enzyme that degrades the incretin hormones GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). By inhibiting DPP-4, vildagliptin increases the levels of GLP-1 and GIP, resulting in elevated fasting and postprandial levels of these hormones. This leads to enhanced insulin production from pancreatic beta cells and decreased glucagon secretion from alpha cells. The increased insulin/glucagon ratio during hyperglycemia results in reduced hepatic glucose production, leading to lower blood sugar levels.

On the other hand, Metformin Hydrochloride, a biguanide class of oral antihyperglycemic medication, has a different mode of action. It reduces both basal and postprandial plasma glucose levels. Unlike sulfonylureas, it does not cause hypoglycemia. Metformin works by reducing hepatic glucose synthesis, decreasing intestinal glucose absorption, and increasing insulin sensitivity. It enhances peripheral glucose uptake and utilization, leading to improved blood sugar control.

Together, vildagliptin and metformin in Viglimet 50/850mg Tablet provide complementary mechanisms of action to effectively manage type 2 diabetes mellitus when diet, exercise, and monotherapy with either vildagliptin or metformin alone are insufficient.

Dosage & Administration of Viglimet 50/850mg Tablet

For adults with Type 2 Diabetes Mellitus, the recommended dose of Viglimet 50/850mg Tablet depends on the patient's current dose of Metformin:

  • The combination of Vildagliptin & Metformin may be initiated at either 50 mg Vildagliptin / 500 mg Metformin or 50 mg Vildagliptin / 850 mg Metformin twice daily.
  • Take 1 tablet in the morning and the other in the evening.
  • The recommended maximum daily dose is 100 mg Vildagliptin plus 2000 mg Metformin Hydrochloride.

For patients with hepatic impairment, including those with a pre-treatment ALT or AST (liver enzymes) >3 times the upper limit of normal, this combination is not recommended.

For children under 18 years of age, Viglimet 50/850mg Tablet is not recommended.

It is essential to take Viglimet 50/850mg Tablet as directed by your physician. The tablet should be taken with or just after food and should be swallowed whole, without crushing or chewing it. The correct dose and duration will be decided by your doctor based on your age and health condition. Do not stop taking Viglimet 50/850mg Tablet without informing your doctor. It is important to follow your doctor's instructions and continue the medication as prescribed for the best results in managing diabetes.
