Pregnancy
CONSULT YOUR DOCTOR

Secrin 2mg Tablet is a pregnancy Category C medicine, indicating that there are no well-controlled studies conducted on pregnant women. If you are taking GLIMEPIRIDE during pregnancy, stop taking it at least 2 weeks before your due date or as prescribed by your doctor.


Breastfeeding
CONSULT YOUR DOCTOR

Not recommended to be taken by breastfeeding mothers. Secrin 2mg Tablet may pass into breast milk and may pose the risk of low blood sugar in the infant. Consult your doctor for further advice.


Driving
CAUTION

Not recommended in patients with fluctuating glucose levels. Glimepiride can reduce the ability to concentrate or react if your blood sugar is lowered (hypoglycemia) or raised (hyperglycemia) or if you develop visual problems as a result of such conditions. Thus, it is not recommended as the patient can endanger himself or others (e.g. when driving a car or using machines).


Kidney

How does Secrin 2mg Tablet work?

Secrin 2mg Tablet acts as an antidiabetic medication by increasing the release of insulin from the pancreas, thereby lowering blood glucose levels.

Side Effects of Secrin 2mg Tablet

Hypoglycemia, headache, nausea, dizziness. Rare: Allergic skin reactions, cholestasis, elevation of liver enzyme levels.

Indications:
  • Glimepiride is recommended as an adjunct to diet and exercise to lower blood glucose in patients with noninsulin-dependent (Type II) diabetes mellitus (NIDDM) when hyperglycemia cannot be controlled by diet and exercise alone.
  • Glimepiride may be used concurrently with metformin when diet, exercise, and either Glimepiride or metformin alone do not result in adequate glycemic control.
  • Glimepiride is also indicated for use in combination with insulin to lower blood glucose in patients whose hyperglycemia cannot be controlled by diet and exercise along with an oral hypoglycemic agent. Combined use of Glimepiride and insulin may increase the potential for hypoglycemia.
Pharmacology

Glimepiride is a sulfonylurea antidiabetic agent that reduces blood glucose concentration. Its primary mechanism of action involves stimulating the release of insulin from functional pancreatic beta cells. Glimepiride acts in concert with glucose by improving the sensitivity of beta cells to physiological glucose stimulation, resulting in insulin secretion. Additionally, extrapancreatic effects such as a reduction in basal hepatic glucose production, increased peripheral tissue sensitivity to insulin, and glucose uptake may also contribute to the action of Glimepiride. In non-fasting diabetic patients, the hypoglycemic activity of a single dose of Glimepiride persists for 24 hours.

Dosage & Administration:

Adult Dose:

  • Initially, 1-2 mg daily, increased in increments of 1-2 mg at intervals of 1-2 weeks. Maintenance: 4 mg daily. Max: 6 mg daily. For the elderly, initially 1 mg once daily. Severe hepatic impairment is contraindicated.

Child Dose:

  • Safety and efficacy not established.

Renal Dose:

  • Renal impairment: 1 mg PO qDay; titrate dose based on fasting blood glucose levels. Severe renal impairment is contraindicated.

Administration:

  • To be taken with food. Should be taken just before or during the first main meal of the day. Avoid skipping meals.
Interaction

Interactions that may potentiate the hypoglycemic activity of Glimepiride include ACE inhibitors, anabolic steroids, azapropazone, chloramphenicol, coumarin derivatives, fluconazole, MAO inhibitors, probenecid, quinolones, salicylates, sulfonamide antibiotics, and more. Drugs that may diminish the hypoglycemic activity include barbiturates, calcium channel blockers, corticosteroids, diazoxide, diuretics, glucagon, isoniazid, nicotinic acid, phenothiazines, rifampicin, sympathomimetic agents, thyroid hormones, and others.

Contraindications

Glimepiride is not suitable for the treatment of insulin-dependent (Type I) diabetes mellitus, diabetic ketoacidosis, or diabetic coma. It should not be used in patients allergic to Glimepiride, other sulfonylureas, other sulfonamides, severe hepatic dysfunction, severe impairment of renal function, or dialysis patients.

Pregnancy & Lactation

Glimepiride is contraindicated during pregnancy, and a transition to insulin is necessary. Breastfeeding women should not take Glimepiride as it may harm the baby. Transitioning or complete cessation of breastfeeding is necessary.

Precaution & Warnings:

Increased risk of CV mortality, monitor blood glucose concentration. Caution in the elderly, hepatic and renal impairment, and SIADH in patients with CHF or hepatic cirrhosis. Pregnancy, lactation.

If a meal is skipped, skip the corresponding dose of Secrin 2. If an extra meal is consumed, an additional dose may be needed.

The patient is advised to talk to a doctor before taking contraceptive pills if he/she is already taking GLIMEPIRIDE, then the doctor may change the dose of GLIMEPIRIDE. This is because oral contraceptive pills change body responds to the sugar.

Avoid intake of Glimepiride and consult a doctor if you have an allergy to sulfa drugs, diabetic ketoacidosis, heart disease, liver or kidney disease, an enzyme deficiency called glucose-6-phosphate dehydrogenase deficiency (G6PD).

The Secrin 2mg Tablet contains Glimepiride which acts on insulin-secreting cells called beta cells inside the pancreas and activates calcium channels on cells which lets insulin pass out of the cell. This insulin then makes each cell in the body take up glucose and utilize it, thus lowering raised blood glucose. Therefore, it prevents symptoms of type 2 diabetes.
