Indication of Piodar 15mg Tablet

Pioglitazone is indicated as an adjunct to diet and exercise to improve glycemic control in patients with type II diabetes mellitus (NIDDM). Pioglitazone is indicated as monotherapy and is also indicated in combination with sulphonylurea, metformin, or insulin when diet, exercise, and drugs alone do not provide adequate glycemic control.

Pharmacology

Pioglitazone acts as a potent and highly selective agonist for the peroxisome proliferator-activated receptor gamma (PPAR-gamma). When these receptors are activated, they stimulate the production of gene products that play a role in lipid and glucose metabolism. Additionally, pioglitazone enhances insulin response in target cells without increasing pancreatic insulin secretion.

Dosage & Administration of

Pioglitazone is typically taken once a day, regardless of meals. Diabetes management should be personalized. It can start at 15 mg or 30 mg once daily for those not well-controlled by diet and exercise alone. If the initial dose isn't effective, it can be increased to 45 mg once daily. For those who don't respond well to monotherapy, combination therapy should be considered. Notably, no placebo-controlled studies have used more than 30 mg once daily in combination.

Pregnancy & Lactation

Pregnancy: There are no adequate and well-controlled studies on pregnant women. Pioglitazone should only be used during pregnancy if the potential benefit compares with the potential risk to the fetus.

Breastfeeding: It is not known whether pioglitazone is excreted in human milk. Since many drugs are excreted in breast milk, the drug should not be given to a nursing woman.

Precautions & Warnings

Pioglitazone's hypoglycemic effect is reliant on the presence of insulin. Thus, it's not suitable for treating type 1 diabetes or diabetic ketoacidosis.
