Pharmacology

Glucagon-like peptide-1 (GLP-1) agonists, which are classified as incretin mimetics, play a crucial role in glucose metabolism by affecting various aspects of pancreatic and gastrointestinal functions. Here's a breakdown of the actions of GLP-1:

Enhanced Glucose-Dependent Insulin Secretion:

  • GLP-1 agonists enhance the secretion of insulin from pancreatic beta-cells.
  • Importantly, this insulin secretion is glucose-dependent. In other words, GLP-1 increases insulin release in response to elevated blood glucose levels, helping to regulate blood sugar.

Suppression of Glucagon Secretion:

  • GLP-1 agonists suppress the release of glucagon from pancreatic alpha-cells.
  • Glucagon typically raises blood glucose levels by stimulating the liver to convert stored glycogen into glucose. By inhibiting this action, GLP-1 helps prevent excessive glucose production.

Slowing Gastric Emptying:

  • GLP-1 agonists slow down the emptying of the stomach contents into the small intestine.
  • This delayed gastric emptying helps to regulate the rate at which nutrients, including glucose, are absorbed from the digestive system into the bloodstream. Slower nutrient absorption contributes to better blood glucose control after meals.

In summary, the actions of GLP-1 agonists contribute to improved glycemic control in individuals with type 2 diabetes. By promoting glucose-dependent insulin secretion, suppressing inappropriate glucagon secretion, and slowing gastric emptying, these medications help maintain more stable blood glucose levels throughout the day. This makes GLP-1 agonists valuable therapeutic options for managing type 2 diabetes, especially when lifestyle modifications and other oral medications are not sufficient.

Dosage

Dosage:

  • 0.25 mg SC week for 4 weeks initially: Start with this lower dosage for the first 4 weeks.
  • Increase to 0.5 mg qWeek: If glycemic control is not achieved after at least 4 weeks on the 0.25 mg dose, increase to this dosage.
  • Further Increase to 1 mg qWeek: If glycemic control is still not achieved after at least 4 weeks on the 0.5 mg dose, the dosage can be increased to 1 mg qWeek.
