Montelukast is used to treat and prevent asthma. It will decrease the symptoms and the number of acute asthma attacks. However, this medicine should not be used to relieve an asthma attack that has already started. This medicine is also used to prevent exercise-induced bronchoconstriction (EIB) and treat symptoms of allergic rhinitis (hay fever), including seasonal (short-term) or perennial (long-term) allergic rhinitides, such as sneezing, runny nose, itching, or wheezing. This medicine is available only with your doctor's prescription.

Pharmacology

Montelukast is an orally active, selective leukotriene receptor antagonist that blocks the cysteinyl leukotriene receptor (CysLT1). The cysteinyl leukotrienes (LTC4, LTD4, LTE4) are arachidonic acid metabolic products that are produced by a variety of cells, including mast cells and eosinophils. Cysteinyl leukotrienes and leukotriene receptor occupancy have been linked to the pathophysiology of asthma and allergic rhinitis, such as airway edema, smooth muscle contraction, and altered cellular activity associated with the inflammatory process, all of which contribute to asthma symptoms.

Dosage & Administration of Montene 10mg Tablet

Adult Dose:

  • Chronic asthma: For adults with chronic asthma, the recommended dose is 10 mg of the film-coated tablet taken once daily in the evening.
  • Allergic rhinitis: Adults with allergic rhinitis should take 10 mg once daily.
  • Prophylaxis of exercise-induced asthma: Adults should take 10 mg at least 2 hours prior to exercise. Additional doses should not be taken within 24 hours.
  • Elderly: No dosage adjustment is needed for elderly patients.
  • Hepatic impairment: For patients with mild to moderate hepatic impairment, no dosage adjustment is needed.

Child Dose:

  • Asthma prophylaxis and maintenance treatment: The dosages for children vary depending on age groups:
    • <12 months: Safety and efficacy not established.
    • 12-24 months: 4 mg (granules) taken orally once daily in the evening.
    • 2-6 years: 4 mg (chewable tablet or granules) taken orally once daily in the evening.
    • 6-15 years: 5 mg (chewable tablet) taken orally once daily in the evening.

15 years: 10 mg (conventional tablet) taken orally once daily in the evening.

  • Exercise-induced bronchospasm prophylaxis:
    • 6-15 years: 5 mg (chewable tablet) taken orally 2 hours before exercise. Additional doses should not be taken within 24 hours.

15 years: 10 mg taken orally 2 hours before exercise. Additional doses should not be taken within 24 hours. If the medication is taken for another indication, an additional dose should not be taken to prevent exercise-induced bronchospasm.

  • Perennial allergic rhinitis:
    • <6 months: Safety and efficacy not established.
    • 6-24 months: 4 mg (granules) taken orally once daily.
    • 2-6 years: 4 mg (chewable tablet or granules) taken orally once daily.
    • 6-15 years: 5 mg (chewable tablet) taken orally once daily.

15 years: 10 mg (conventional tablet) taken orally once daily.

  • Seasonal allergic rhinitis:

Renal Dose:

  • Renal impairment: No dosage adjustment is needed for patients with renal impairment.

Interaction
  • The plasma concentration of the medication may be decreased when used together with potent inducers of cytochrome P450 isoenzymes, such as phenobarbital, phenytoin, and rifampicin. 
  • When combined with theophylline, prednisone, prednisolone, terfenadine, digoxin, warfarin, gemfibrozil, itraconazole, thyroid hormones, sedative-hypnotics, nonsteroidal anti-inflammatory agents, benzodiazepines, decongestants, oral contraceptives, and cytochrome P450 (CYP) enzyme inducers, no dose adjustment is required.
  • The medication's bioavailability and effectiveness are not significantly affected by food or other circumstances.
