  • Maintenance of healing of erosive esophagitis
  • Symptomatic Gastroesophageal Reflux Disease (GERD)
  • Reduction of NSAID-associated gastric ulcer
  • Pylori eradication (Triple therapy)
  • Zollinger-Ellison syndrome
Pharmacology

The H+/K+ ATPase in the gastric parietal cell is inhibited by the proton pump inhibitor esomeprazole, which reduces the amount of stomach acid produced. Esomeprazole (omeprazole's S-isomer), the first proton pump inhibitor with a single optical isomer, offers better acid control than racemic proton pump inhibitors.

Dosage & Administration of Maxpro 40mg Tablet

Adult Dose:

  1. GERD Without Erosive Esophagitis: 20 mg orally once a day for 4 weeks. Consider an additional 4 weeks of treatment if symptoms do not completely resolve in the first 4 weeks.
  2. GERD With Erosive Esophagitis: 20-40 mg orally once a day for 4-8 weeks. If oral therapy is inappropriate or not possible, 20-40 mg intravenously once a day for up to 10 days. Switch to oral administration once the patient is able to swallow. The maintenance dose is 20 mg orally once a day for up to 6 months.
  3. Risk Reduction of NSAID-Associated Gastric Ulcer: 20-40 mg orally once a day for up to 6 months.
  4. NSAID-Induced Gastric Ulcer: 20 mg orally once a day for 4-8 weeks.
  5. Zollinger-Ellison Syndrome: Initial dose of 80 mg orally divided every 12 hours, adjust the regimen based on efficacy. The maximum dose is up to 240 mg orally once a day or 120 mg orally every 12 hours.

Child Dose: The dosing for children varies based on the condition and age. Here are some examples:

  1. GERD Without Erosive Esophagitis:
    • 1-12 years: 10-20 mg orally once a day for up to 8 weeks.

12 years: 20-40 mg orally once a day for up to 8 weeks.

  1. GERD With Erosive Esophagitis (Healing):
    • 1 month to 1 year:
      • <3.5 kg: 2.5 mg orally once a day for up to 6 weeks.

3.5-7.5 kg: 5 mg orally once a day for up to 6 weeks.

7.5 kg: 10 mg orally once a day for up to 6 weeks.

  • 1-12 years:
    • <20 kg: 10 mg orally once a day for 8 weeks.

20 kg: 10-20 mg orally once a day for 8 weeks.

12 years: 20-40 mg orally once a day for 4-8 weeks. Maintenance dose is 20 mg orally once a day for up to 6 months.

Renal Dose: No dosage adjustment is needed for patients with renal impairment.

Interaction
  • Digoxin: Esomeprazole may increase the risk of digoxin-induced cardiotoxic effects.
  • Diuretics: There is an increased risk of hypomagnesemia (low magnesium levels) when Esomeprazole is used with diuretics.
  • Warfarin: Esomeprazole may increase the International Normalized Ratio (INR) and prothrombin time when used concomitantly with warfarin, which may increase the risk of bleeding.
  • Tacrolimus, saquinavir, methotrexate: Esomeprazole may increase the serum concentration of these drugs, potentially leading to an increased risk of adverse effects.
  • Drugs metabolized by CYP2C19 (e.g., diazepam): Esomeprazole may interfere with the elimination of drugs metabolized by the CYP2C19 enzyme, such as diazepam.
