Indications

Rivastigmine is prescribed for the treatment of mild to moderately severe symptoms of Alzheimer's disease and, in individuals with idiopathic Parkinson's disease, for symptomatic therapy of mild to moderately severe dementia.

Pharmacology

Rivastigmine, a carbamate derivative, inhibits cholinesterases, primarily acetylcholinesterase, and butyrylcholinesterase, increasing acetylcholine content at cholinergic synapses. Its action is relatively selective for brain cholinesterases compared to those in peripheral tissues.

Dosage & Administration:

Rivastigmine Transdermal Patch:

• Initial dose: 4.6 mg/24 hours patch.
• Dose titration: After a minimum of 4 weeks, increase to 9.5 mg/24 hours, then to a maximum of 13.3 mg/24 hours.
• Alzheimer's and Parkinson’s Disease Dementia: 9.5 mg/24 hours or 13.3 mg/24 hours once daily.
• Severe Alzheimer’s Disease: 13.3 mg/24 hours once daily.
• For treatment interruption longer than 3 days, reiterate starting at 4.6 mg/24 hours.

Rivastigmine should be administered twice a day, with morning and evening meals.

Interaction

Rivastigmine may interact with succinylcholine-type muscle relaxants during anesthesia and other cholinergic substances, potentially affecting anticholinergic drugs.

Contraindications

Contraindicated in patients hypersensitive to the active substance or other carbamate derivatives.

Pregnancy & Lactation

Limited clinical data exist. Use during pregnancy is only recommended if necessary. Its secretion in milk suggests caution during lactation.

Precautions & Warnings

Caution is advised in patients with various conditions, including sick sinus syndrome, conduction abnormalities, respiratory disorders, urinary obstruction, convulsions, pregnancy, renal failure, hepatic failure, asthma, or obstructive pulmonary disease.
