Bisoprolol Fumarate is a highly selective ß1 adrenergic receptor blocker, displaying the greatest affinity for ß1 receptors compared to other beta-blockers. It selectively inhibits ß1 receptors in the heart and vascular smooth muscle, reducing heart rate and cardiac output, which lowers arterial blood pressure. Unlike non-selective ß-blockers, Bisoprolol does not adversely affect lipid metabolism or HDL-cholesterol levels during long-term use.
The pharmacokinetics of Bisoprolol allow for once-daily dosing, ensuring consistent plasma concentration levels with minimal variability.
- Absorption & Bioavailability: Over 90% of Bisoprolol is absorbed from the gastrointestinal tract, with an absolute bioavailability of 88% due to minimal first-pass metabolism (<10%). Food does not affect its absorption.
- Distribution: Bisoprolol is widely distributed, with a medium distribution volume of 3.5 L/kg.
- Metabolism: It undergoes oxidative metabolism without conjugation, and the resulting polar metabolites are excreted renally. The primary metabolic pathway involves CYP3A4 (~95%), with minor involvement of CYP2D6.
- Elimination: Bisoprolol clearance is balanced between renal excretion of the unchanged drug (~50%) and hepatic metabolism (~50%). The elimination half-life is 10-12 hours.

✔️ Pregnancy & Lactation
Pregnancy:
- Studies in animals suggest that Bisoprolol is not teratogenic at doses up to 375 times the maximum human dose. However, it showed fetal toxicity at higher doses.
- Use during pregnancy only if the potential benefit outweighs the risk to the fetus.
Lactation:
- Bisoprolol passes into animal milk in small amounts (<2%). It is unclear if it is excreted in human milk. Nursing mothers should discontinue breastfeeding if Bisoprolol use is necessary.

Adults:
For mild to moderate hypertension, the dosage should be tailored to the individual:
- Start with 5 mg once daily, either alone or combined with a diuretic.

Children:

Renal or Hepatic Impairment:
- For patients with liver dysfunction (e.g., hepatitis or cirrhosis) or renal impairment (creatinine clearance <40 mL/min), start with 5 mg daily.
- Titrate doses cautiously to avoid drug accumulation. Bisoprolol is not dialyzable, so replacement is unnecessary for dialysis patients.
Elderly:
Dose adjustments are generally not required unless significant renal or hepatic impairment is present.

- Centrally Acting Antihypertensives: Caution is needed when combining Bisocor with drugs like clonidine due to the risk of rebound hypertension. If discontinuing clonidine, stop Bisocor several days before to avoid exacerbation.
- Antiarrhythmic Agents: Use Bisocor carefully with myocardial depressants or A-V conduction inhibitors, such as certain calcium channel blockers (e.g., verapamil, diltiazem) or antiarrhythmics (e.g., disopyramide).
- Calcium Channel Blockers: The combination of Bisocor and calcium channel blockers with negative inotropic effects may lead to bradycardia, severe hypotension, or cardiac failure in patients with impaired ventricular function.
