Indications
This tablet is used to treat Parkinson's disease and Parkinson's syndrome. It helps to alleviate many of the symptoms of Parkinson's disease, especially rigidity and bradykinesia. Tremor, dysphagia, sialorrhea, and postural instability are common symptoms of Parkinson's disease and syndrome, and this tablet can assist.
Pharmacology
Levodopa is the metabolic precursor of dopamine. It crosses the blood-brain barrier and is converted to dopamine in the brain. Carbidopa increases the amount of levodopa transported to the central nervous system by inhibiting the decarboxylation of peripheral levodopa. Entacapone is a selective COMT inhibitor that can change the pharmacokinetics of levodopa, resulting in higher and sustained serum levels of levodopa.
Dosage & Administration
Adults: The optimum daily dose must be determined by careful titration of levodopa in each patient. Patients should be instructed to take only 1 (one) tablet per dose administration. The maximum recommended daily dose of entacapone is 2,000 mg. Usually this combination is to be used in patients who are currently treated with corresponding doses of standard release Levodopa or Dopa Decarboxylase (DDC) inhibitor and entacapone.
May be taken with or without food. Keep a consistent diet. A change in diet to foods high in protein may delay levodopa absorption & reduce amount taken up in circulation. Excessive acidity also delays stomach emptying & thus delays levodopa absorption. Iron salts may also reduce amount of levodopa available to the body. Swallow whole.
Interactions
When levodopa is added to the treatment of patients already receiving antihypertensive drugs, symptomatic orthostatic hypotension may occur. The dosage of antihypertensive drugs may need to be adjusted. Dopamine receptor antagonists (such as some antipsychotics and antiemetics), phenytoin, and papaverine can reduce the therapeutic effect of levodopa. Patients taking these drugs in combination with levodopa/carbidopa/entacapone should be carefully monitored for loss of treatment response. Because levodopa competes with certain amino acids, some patients on a high-protein diet may affect the absorption of levodopa, carbidopa, and entacapone.
Contraindications
History of narrow-angle glaucoma, pheochromocytoma, antipsychotic malignant syndrome (NMS), and/or non-traumatic rhabdomyolysis. Use at the same time for severe liver failure or within 14 days after stopping non-selective MAOI.
Side Effects
Common side effects include dyskinesia, nausea, excessive exercise, changes in urine color, diarrhea, and stomach pain. Other side effects may include diarrhea, sometimes severe; colitis; hallucinations and other mental disorders; orthostatic hypotension; rhabdomyolysis; and symptoms similar to antipsychotic malignant syndrome (a type characterized by high fever, muscle stiffness, and confusion Disease); fibrosis; skin cancer, etc.
Pregnancy & Lactation
Pregnancy category C. The levodopa/carbidopa/entacapone combination should not be used during pregnancy unless the potential benefit justifies the potential risk to the fetus. The safety of this combination in nursing infants is unclear. During the use of this combination therapy, women should not breastfeed.
Precautions & Warnings
The combination of levodopa, carbidopa and entacapone can cause dizziness and symptomatic erection. Therefore, you should be cautious when driving or using machines. As with levodopa, it is recommended to regularly evaluate liver, hematopoietic, cardiovascular, and renal functions during long-term treatment.
