Pharmacology

Naproxen, a propionic acid derivative, is a typical NSAID. It works by reversibly inhibiting the cyclooxygenase-1 and -2 (COX-1 and -2) enzymes, leading to decreased synthesis of prostaglandin precursors. It also has the ability to inhibit platelet aggregation and possesses anti-inflammatory, analgesic, and antipyretic properties.

Dosage & Administration of Naproxen 500mg Tablet

Adult Dose:

  • Pain: Initially 500 mg PO, then 250 mg PO q6-8hr or 500 mg PO q12hr PRN; not to exceed 1250 mg/day naproxen base on day 1; subsequent daily doses should not exceed 1000 mg naproxen base.
  • Rheumatoid Arthritis, Osteoarthritis, Ankylosing Spondylitis: 500-1000 mg/day PO divided q12hr; may increase to 1500 mg/day if tolerated well for a limited time.
  • Dysmenorrhea: Initially 500 mg PO, then 250 mg PO q6-8hr or 500 mg PO q12hr (long-acting formula); not to exceed 1250 mg/day on the first day; subsequent doses should not exceed 1000 mg/day naproxen base.
  • Gout, Acute: Initially 750 mg PO, followed by 250 mg q8hr until the attack subsides; extended-release: 1000-1500 mg qDay, followed by 1000 mg qDay until the attack subsides.

Child Dose:

  • Pain >12 years: Initially 500 mg PO, then 250 mg PO q6-8hr or 500 mg PO q12hr PRN; not to exceed 1250 mg/day naproxen base on day 1; subsequent daily doses should not exceed 1000 mg naproxen base.
  • Juvenile Idiopathic Arthritis >2 years: 10 mg/kg/day oral suspension PO divided q12hr; not to exceed 15 mg/kg/day.

Renal Dose:

  • Renal impairment with CrCl <30 mL/min: Use not recommended.
  • Severe renal impairment: Contraindicated.
Interaction

Drug Interactions:

  • Increased toxicity: Naproxen 500mg Tablet may interact with other highly protein-bound drugs such as oral anticoagulants, hydantoins, salicylates, sulfonamides, and sulfonylureas.
  • Naproxen 500mg Tablet and warfarin may cause a slight increase in free warfarin.
  • Naproxen 500mg Tablet and probenecid may cause an increased plasma half-life of naproxen.
  • Naproxen 500mg Tablet and methotrexate may significantly increase and prolong blood methotrexate concentration, which may be severe or fatal.

Interaction with Alcohol:

  • Alcohol consumption is not advised while taking this medication as it can result in stomach bleeding.

Interaction with Disease:

  • Use with caution in patients with GI disease (bleeding or ulcers), cardiovascular disease (CHF, hypertension), dehydration, renal or hepatic impairment, and patients receiving anticoagulants.
  • Perform ophthalmologic evaluation for those who develop eye complaints during therapy (blurred vision, diminished vision, changes in color vision, retinal changes).
  • NSAIDs may mask signs/symptoms of infections.
  • Photosensitivity reported; elderly are at especially high risk for adverse effects.

Interaction with Food:

  • Food may decrease the rate but not the extent of oral absorption.
  • Drugs may cause GI upset, bleeding, ulceration, and perforation; take with food or milk to minimize GI upset.

  • Known hypersensitivity to naproxen.
  • History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
