The fungi gain entry into the nail through small openings or cracks in the nail or surrounding skin, leading to an infection. Symptoms of fungal nail infections often include thickened nails that may appear discolored (brown, yellow, or white), making them more prone to breakage. While these infections are generally not painful unless they become severe, they can be quite persistent. Fungal infections are more commonly observed in toenails than in fingernails.

Tinea capitis, commonly referred to as scalp ringworm, is another fungal infection that primarily affects the scalp, involving both the hair and the skin. This infection is contagious and manifests with symptoms such as dry, itchy, scaly patches on the scalp, hair loss, and redness. The term "ringworm" is used because of the circular or ring-shaped appearance of the affected areas. Tinea capitis predominantly occurs in children between the ages of 3 to 7 years.

Pharmacology
  • Mycofree 250 contains Terbinafine, an Allylamine antifungal, which inhibits Squalene Epoxidase, suppressing the biosynthesis of Ergosterol, a crucial component of fungal cell membranes.
  • It may be fungicidal based on drug concentration and in vitro testing against fungal species like Trichophyton Mentagrophyte and Trichophyton rubrum.
Dosage & Administration of Mycofree 250mg Tablet

Adult Dose:

Oral Onychomycosis:

  • Fingernail: 250 mg once daily for 6 weeks
  • Toenail: 250 mg once daily for 12 weeks

Tinea Pedis, Tinea Corporis, Tinea Cruris:

  • 250 mg/day orally in a single dose or divided into two doses every 12 hours
  • Treatment duration varies depending on the type of fungal infection: 2-4 weeks for tinea cruris and tinea corporis, and 2-6 weeks for tinea pedis

Hepatic Impairment: Terbinafine use is not recommended in individuals with chronic or active liver disease.

Child Dose:

Tinea Capitis:

  • Children >4 years and weighing less than 25 kg: 125 mg/day orally for 6 weeks
  • Children >4 years and weighing 25-35 kg: 187.5 mg/day orally for 6 weeks
  • Children >4 years and weighing more than 35 kg: 250 mg/day orally for 6 weeks

Renal Dose:

Renal Impairment: Terbinafine use is not recommended if the creatinine clearance (CrCl) is less than 50 mL/min.
