Dosage and Administration of Flamyd 500mg Tablet

Adult Dose:

  • For amoebiasis (intestinal infection): Take 800 mg three times a day for 5 days. The maximum daily dose is 2.4 g.
  • For amoebiasis (extra-intestinal infection): Take 800 mg three times a day for 5-10 days. The maximum daily dose is 2.4 g.
  • For trichomoniasis: Take 2 g as a single dose, or 200 mg three times a day for 7 days, or 400 mg twice a day for 5-7 days. Sexual partners should also be treated. Repeat treatment if necessary after 4-6 weeks.
  • For giardiasis: Take 2 g once daily for 3 days, or 400 mg three times a day for 5 days, or 500 mg twice a day for 7-10 days.
  • For bacterial vaginosis: Take 2 g as a single dose, or 400 mg twice a day for 5-7 days.
  • For acute necrotizing ulcerative gingivitis: Take 200 mg three times a day for 3 days.
  • For anaerobic bacterial infections: Initially, take 800 mg followed by 400 mg every 8 hours, or alternatively, take 7.5 mg/kg every 6-8 hours. The maximum daily dose is 4 g. The duration of treatment is usually 7 days depending on the severity of the infection.
  • For prophylaxis of postoperative anaerobic bacterial infections: Take 400 mg every 8 hours, starting 24 hours before surgery, and switch to oral therapy when possible.
  • For eradication of H. pylori associated with peptic ulcer disease: Take 400 mg twice a day in combination with other antibacterial agents and a proton pump inhibitor (PPI), or take 400 mg three times a day if given with omeprazole and amoxicillin. The initial treatment is given for 1 week.
  • For leg ulcers and pressure sores: Take 400 mg three times a day for 7 days.
  • For acute dental infections: Take 200 mg three times a day for 3-7 days.

Child Dose:

  • For amoebiasis: Children aged 1-3 years should take 100-200 mg three times a day, children aged >3-7 years should take 100-200 mg four times daily, and children aged >7-10 years should take 200-400 mg three times a day. The duration of treatment is 5-10 days.
  • For trichomoniasis: Children aged 1-10 years should take 40 mg/kg as a single dose or 15-30 mg/kg daily in 2-3 divided doses for 7 days. The maximum dose is 2 g.
  • For giardiasis: Children aged 1-3 years should take 500 mg once daily, children aged >3-7 years should take 600-800 mg once daily, and children aged >7-10 years should take 1 g once daily. The duration of treatment is 3 days.
  • For acute necrotizing ulcerative gingivitis: Children aged 1-3 years should take 50 mg three times a day, children aged >3-7 years should take 100 mg twice a day, and children aged >7-10 years should take 100 mg three times a day. The duration of treatment is 3 days.
  • For anaerobic bacterial infections: Infants younger than 8 weeks should take 7.5 mg/kg every 12 hours
Interaction
  • When metronidazole is used in conjunction with disulfiram (a medication used in the treatment of alcoholism), it may lead to psychotic responses. Therefore, caution should be exercised when using these medications together.
  • Metronidazole can enhance the effects of oral anticoagulants (medications that prevent blood clotting), potentially increasing the risk of bleeding. Close monitoring of coagulation parameters is advised when these medications are used concurrently.

  • The use of metronidazole with busulfan (a chemotherapy medication) can cause a significant increase in busulfan plasma levels, leading to severe toxicity. Caution should be exercised, and close monitoring of busulfan levels and patient response is essential in such cases.
  • Phenobarbital and phenytoin are medications used for epilepsy and seizures. When used concomitantly with metronidazole, they can enhance the metabolism of metronidazole, resulting in reduced serum concentrations. Monitoring of metronidazole levels and adjustment of the dosage may be necessary in patients using these medications together.
Contraindications

Metronidazole is contraindicated in patients with a history of hypersensitivity to Metronidazole or other Nitroimidazole derivatives.

Pregnancy & Lactation

Metronidazole is classified as a pregnancy category B by the US Food and Drug Administration. However, no appropriate and well-controlled trials in pregnant women have been conducted. Because animal reproduction studies do not always anticipate a human response, this medication should only be taken during pregnancy if absolutely necessary. Human milk has been found to contain metronidazole. When Metronidazole is given to a breastfeeding mother, care should be used.
