Description:

Erian Ointment (Cinchocaine HCI + Hydrocortisone + Framycetin Sulphate + Esculin) is a medication used to treat anal fissures and hemorrhoids. It provides relief from pain and itching caused by inflammation. The ointment works by numbing the area where it is applied, reducing discomfort in and around the anal region.

  • It should only be used externally as directed by your doctor. Using too much or too little of the ointment can have adverse effects or reduce its effectiveness. Follow your doctor's prescription and continue using the medication for the recommended duration. Avoid contact with your eyes, mouth, or open wounds. If accidental contact occurs, rinse the affected area with water immediately.
  • Possible side effects of Erian Ointment include temporary irritation, redness, itching, or a burning sensation at the site of application. These side effects typically resolve on their own without treatment. However, if they persist or worsen, inform your doctor.
  • Inform your doctor if you have a history of allergic reactions to any components of the ointment. Avoid using Pio Ointment if you have a systemic infection. It is important to disclose any other medications you are taking to ensure the safety of using this ointment. Pregnant and breastfeeding women should consult their doctors before using Erian Ointment.

  • Long-term and continuous use of topical corticosteroids should be minimized due to the risk of skin atrophy, including thinning, loss of elasticity, dilation of superficial blood vessels, telangiectasia, and ecchymoses. These changes are more likely to occur when occlusive dressings are used. Systemic absorption of corticosteroids can occur with high doses, extensive application, application to damaged skin, use of potent corticosteroids, and occlusive dressing techniques. This can lead to hypothalamic-pituitary-adrenal axis depression, resulting in adrenal gland suppression. These effects are more severe in children and can cause growth retardation and a Cushingoid state. Rare cases of benign intracranial hypertension have also been reported.
How Erian Ointment work?

Cinchocaine works by preventing or reducing the conduction of nerve impulses near the site of administration. It achieves this by decreasing the permeability of nerve cells to sodium ions, which helps in blocking the initiation and transmission of pain signals.

Hydrocortisone, a corticosteroid, is known for its anti-inflammatory and immunosuppressive properties. Its anti-inflammatory action is attributed to the inhibition of leukocyte migration and the reversal of increased capillary permeability. Hydrocortisone can also be used as a replacement therapy in cases of adrenocortical insufficiency.

Framycetin sulfate is a broad-spectrum antibiotic that is effective against various types of infections. In the context of anorectal lesions, it helps in eradicating most infections that may already be present or may arise in that area.

Esculin, another component, improves skin vasculature and has proven efficacy in managing conditions such as cellulitis, which involves inflammation and infection of the skin and underlying tissues.

Quick Suggestions
  • Wash your hands before and after applying Erian Ointment.
  • Do not take any oral laxatives along with Erian Ointment without consulting your doctor.
  • Sometimes certain medicines like iron tablets and antibiotics might cause constipation. Inform your doctor if you are taking any of them.
  • A fiber-rich diet (wholegrain bread and cereals, fruit, and green leafy vegetables), six to eight glasses of water each day, and daily exercise are important in maintaining healthy bowel function.
  • Apart from using Erian Ointment, you can indulge in warm baths. It may help soothe the area, thus easing the pain.
  • Don't ignore the feeling of needing to pass stools. Some people suppress this feeling and put off going to the toilet until later. This may result in bigger and harder stools forming that are more difficult to pass later.

Pharmacology

Cinchocaine Hydrochloride, a local anesthetic, helps prevent or relieve severe pain caused by conditions such as strangulated hemorrhoids, fissures, and perianal hematomas. Hydrocortisone, a corticosteroid, acts as a decongestant, anti-inflammatory, and anti-pruritic agent. It reduces itching, inflammation, and mucous discharge. Framycetin Sulphate, a broad-spectrum antibiotic, helps eradicate any existing or emerging infections in the anorectal area. Esculin has a skin-protecting function, providing additional protection and support.

Dosage and Administration of Erian Ointment

The ointment should be applied in a small amount using the finger, directly on the painful or itchy area. This should be done in the morning and evening, as well as after each bowel movement. For deeper application, you can attach the supplied applicator/cannula to the tube, insert it fully, and gently squeeze the tube from the lower end while withdrawing.

For adults, the recommended dose is to apply the ointment onto the affected area in the morning and evening, as well as after each bowel movement. If using suppositories, insert one suppository in the morning and evening, and after each bowel movement.

Contraindications

Animal studies have shown that topical application of corticosteroids can be teratogenic. Since these substances can be absorbed through the skin, the possibility of teratogenic effects from topical treatment cannot be excluded. Therefore, pregnant women should avoid using this product. Topical corticosteroid formulations are contraindicated in the treatment of herpes simplex, vaccinia or varicella, or tuberculous infection of the anal region.

Pregnancy & Lactation

The safety of using topical corticosteroids during pregnancy has yet to be determined. As a result, they should not be used in wide areas, in big volumes, or for lengthy periods of time during pregnancy.

Precautions & Warnings
  • Discontinue use of the product if any sensitivity or allergic reactions occur.
  • It is important to address any underlying infections, allergies, or underlying causes appropriately.
  • Although the chances are minimal, long-term use of this product may potentially lead to systemic corticosteroid effects.
  • Patients should inform their future healthcare providers about their previous use of hydrocortisone.

Frequently Asked Questions (FAQ)

Framycetin Sulphate, a broad-spectrum antibiotic, is effective in eliminating various infections that may be present or develop in anorectal lesions. Esculin, on the other hand, improves skin vasculature and has shown efficacy in managing cellulitis. However, if there is a known hypersensitivity or allergy to any of the four ingredients in the medication, its use should be avoided. It's important to inform your healthcare provider about any known allergies before using the medication.

This ointment is often required for at least 6 weeks, or until the fissure is fully healed.

Piles (hemorrhoids), fissures, and fistulas are three distinct conditions related to the anal area. Here are the differences between them: Piles (Hemorrhoids): Piles are swollen blood vessels in the rectum or anus. They can be internal (inside the rectum) or external (around the anus). Piles can cause discomfort, itching, and sometimes bleeding during bowel movements. They are often painless and may go unnoticed unless they become enlarged or irritated. Fissures: Fissures are small tears or cracks in the lining of the anus. They typically occur due to trauma or injury during bowel movements. Fissures can cause severe pain and discomfort, especially during and after bowel movements. They may also lead to bleeding and itching in the anal area. Fistulas: Fistulas are abnormal passages that develop between an internal organ or blood vessel and the skin near the anus. In the case of anal fistulas, they usually connect the anal canal or rectum to the skin around the anus. Fistulas often result from an infection or abscess in the anal area. They can cause persistent drainage of pus or other fluids, along with discomfort and pain. In summary, while piles are swollen blood vessels, fissures are cracks or tears, and fistulas are abnormal passages connecting the anal area with surrounding tissues. Piles are typically painless, fissures cause pain, and fistulas involve the discharge of pus. It's important to consult a healthcare professional for an accurate diagnosis and appropriate treatment for these conditions.
