Bexitrol F 50/250 Bexicap is a combination of two medicines namely, Fluticasone and Salmeterol. Fluticasone is an inhalation corticosteroid. It blocks the release of certain chemical messengers in the airways (such as prostaglandins, leukotrienes) that cause inflammation and swelling.

Salmeterol is a bronchodilator. It relaxes the smooth muscles of the airways and widens them. Collectively, they reduce airway inflammation, allow the air to pass through more freely, and make breathing easier.

Indication of Bexitrol F 50/250 Bexicap

Bexitrol F 50/250 Bexicap is a combination medicine containing Fluticasone and Salmeterol, which belongs to the groups of Corticosteroids and Bronchodilators respectively. It is used to treat asthma and COPD and also helps to reduce the flare-ups of COPD symptoms.

Asthma is a respiratory disease that causes the airways to become inflamed, swollen, and narrow, making it difficult for air to pass through. This results in symptoms like wheezing, breathing difficulties, chest pain, and coughing with excessive mucus secretion in the airways.

COPD is a lung disease that causes chronic inflammation and obstructed airflow in the airways, leading to symptoms such as wheezing, breathing difficulties, chest pain, and coughing with mucus production.

Pharmacology

Salmeterol Xinafoate is a medication that belongs to the class of long-acting, selective beta-2 agonists. It is primarily used to treat asthma and other forms of airway obstruction. On the other hand, Fluticasone Propionate is a glucocorticoid and a type of corticosteroid that has primarily topical effects on the lungs at the recommended dosage.

Fluticasone Propionate helps in improving lung function and preventing exacerbations of the disease, while Salmeterol works to prevent symptoms. This combination of medications may provide a more convenient treatment regimen for patients who require concurrent therapy with a beta-2 agonist and inhaled corticosteroid.

The mechanism of action of Salmeterol involves binding to the exo-site of the beta-2-adrenoceptor, which is a selective, long-acting (12-hour) agonist.

Dosage & Administration of Bexitrol F 50/250 Bexicap

The recommended dosages for inhalation aerosol and powder in capsule or Maxhaler forms vary depending on the age and condition of the patient. For adolescents and adults, the inhalation aerosol can be taken in 2 puffs of either 25g salmeterol and 50g fluticasone propionate, 25g salmeterol and 125g fluticasone propionate, or 25g salmeterol and 250g fluticasone propionate, twice daily. Children aged 4 to 12 can take 2 puffs of 25g salmeterol and 50g fluticasone propionate twice daily.

For inhalation powder in capsule form, the recommended dosages for adults and adolescents (12 years and older) are twice daily of either salmeterol 50g and fluticasone 100g or salmeterol 50g and fluticasone 250g, based on the severity of the patient's asthma. The maximum recommended dosage is salmeterol 50g and fluticasone 500g twice daily. For pediatric patients aged 4 to 11 with asthma, the recommended dosage is salmeterol 50g and fluticasone 100g twice daily.

For inhalation powder in Maxhaler form, the recommended dosages vary depending on the patient's age and condition. For adults and adolescents (12 years and older), one inhalation of either 50/100, 50/250, or 50/500 Maxhaler is recommended twice daily. For children aged 4 years and older, one inhalation of 50/100 Maxhaler twice daily is recommended. For COPD patients, one inhalation of Maxhaler twice daily is recommended.

Special patient groups: There is no need to adjust the dose in elderly or renal-impaired patients. There are no data on its use in patients with hepatic impairment. The Maxhaler is a user-friendly, ready-to-use, and easy-to-grip device. Follow the directions on the package.

Administration

• Although using an inhaler appears to be simple, most patients do not know how to use it correctly. When the inhaler is used incorrectly, less medicine reaches the lungs. Correct and consistent inhaler use will prevent or lessen the severity of asthma attacks.
• According to the Asthma Association's "National Asthma Guidelines for Medical Practitioners," the following simple steps can help you use your inhaler more effectively:
• Remove the cap.
• Shake the inhaler vigorously (at least six times) before each use.
• If the inhaler is new or hasn't been used in a week or more, shake it vigorously and take one puff into the air to ensure that it works.
• Breathe out as fully as you can comfortably and keep the inhaler upright.
• Insert the actuator between your teeth and close your lips around the mouthpiece.
• To release medicine, press down firmly and slowly through the mouth while breathing deeply and slowly.
• Take the inhaler out of your mouth. Continue holding your breath for at least 10 seconds, or for as long as you are comfortable.
• Wait 1 minute between puffs if your doctor has prescribed more than one inhalation per treatment (inhalations). Shake the inhaler vigorously and repeat steps 4–7.
• Replace the mouthpiece's cap after use. Rinse your mouth with water after each treatment.
• Check your technique in front of a mirror on a regular basis; if you see a white mist during inhalation, you may not have properly closed your lips around the mouthpiece, or you may not be breathing in as you press the can. This indicates a technique failure. If this occurs, carefully repeat the procedure from step 4.
• Cleaning Instructions: Clean your inhaler at least once a week. Remove the canister and rinse the plastic actuator and cap in warm water, but do not immerse the metal canister. After thoroughly drying the actuator and cap, gently twist the metal canister into the actuator. Put the mouthpiece cap on.

Interaction

Non-selective and selective -blockers should be avoided in asthma patients unless absolutely necessary. Because of the relatively low plasma concentrations obtained following inhalation treatment, clinically relevant medication interactions are unlikely. Fluticasone Propionate should be used with caution when combined with known strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir) because there is a risk of increased systemic Fluticasone Propionate exposure.

Contraindications

This is not recommended for people who have had a history of hypersensitivity to any of the components.

Pregnancy & Lactation

Drug administration during pregnancy and breastfeeding should be undertaken only if the predicted benefit to the mother outweighs any potential harm to the fetus or child. There is inadequate data on the usage of Salmeterol Xinafoate and Fluticasone Propionate during pregnancy and lactation in humans. There is no information available about human breast milk.

Precautions & Warnings

• If an infection is present, additional corticosteroid treatments, as well as antibiotic medication, should be considered.
• In patients with active or dormant pulmonary tuberculosis, this formulation, like all inhaled corticosteroid medications, should be used with caution.
• This medication should be used with caution in people who have thyrotoxicosis.
