✔️ Dosage & Administration: Oral Formulation

To be taken 1 hour before or after meals

≥40 kg body weight

Loading dose: 400 mg (10 ml) every 12 hours for the first 24 hours

Maintenance dose: 200 mg (5 ml) twice daily

<40 kg body weight

Loading dose: 200 mg (5 ml) every 12 hours for the first 24 hours

Maintenance dose: 100 mg (2.5 ml) twice daily

Or as prescribed by a registered physician

✔️ Drug Interactions

• Inhibitors or inducers of CYP3A4, CYP2C9, and CYP2C19 may affect voriconazole levels. Monitor for toxicity or reduced efficacy.
• May increase blood levels of drugs metabolized by CYP enzymes — dose adjustment required.
• Phenytoin or Efavirenz: Voriconazole dose may need to be increased.
• Avoid co-administration with:
• Terfenadine, cisapride, pimozide, quinidine, astemizole (risk of QT prolongation)
• Rifampin, carbamazepine, long-acting barbiturates, ritonavir, ergot alkaloids, St. John's Wort

Others: Anemia, chest pain

Known hypersensitivity to voriconazole or any component

Concurrent use of medications listed above may cause life-threatening adverse reactions

✔️ Warnings & Precautions

Use with caution in long-term therapy (≥180 days); increased risk of cutaneous squamous cell carcinoma (SCC) has been reported.

Regular monitoring of liver function, visual disturbances, and dermatologic reactions is advised.

Adjust dose and monitor with CYP-metabolized drugs.

Use caution in hepatic and renal dysfunction.
