• Scedosporium and Fusarium spp.

General Guidelines:

Should be taken on an empty stomach (1 hour before or after meals).

IV infusions should be administered over 1–2 hours, not exceeding 3 mg/kg/hour.

Adult Dosage:

• Invasive Aspergillosis, Candidemia, Serious Fungal Infections:
• Loading dose: 6 mg/kg IV every 12 hours for the first 24 hours
• Maintenance dose: 4 mg/kg IV every 12 hours or 200 mg orally every 12 hours
• Esophageal Candidiasis (including Candida albicans, glabrata, krusei):
• 200 mg orally every 12 hours
• Hepatic Impairment (Child-Pugh A & B):
• Standard loading dose
• Maintenance dose reduced by 50%

Pediatric Dosage (≥12 years):

Same as adult dosage

<12 years: Safety and efficacy have not been established

• Known hypersensitivity to voriconazole

Concurrent use with certain drugs due to risk of serious adverse effects or reduced efficacy, including:

• CYP3A4 substrates: terfenadine, astemizole, cisapride, pimozide, quinidine
• Enzyme inducers: rifampicin, carbamazepine, phenobarbital
• Antiretrovirals: ritonavir (≥400 mg bid), efavirenz (≥400 mg daily)
• Ergot alkaloids (e.g., ergotamine)
• St. John's Wort
• Sirolimus and rifabutin

Increased Levels of Other Drugs:

• Immunosuppressants: Cyclosporine, tacrolimus
• Opiates: Oxycodone, methadone, fentanyl, alfentanil
• NSAIDs: Ibuprofen, diclofenac

• Oral contraceptives

Reduced Voriconazole Levels (or vice versa):

• Phenytoin, rifampicin, carbamazepine, phenobarbital, efavirenz, ritonavir, rifabutin

Potentially Fatal Interactions:

• Risk of QT prolongation or torsades de pointes with:
• Terfenadine, astemizole, cisapride, quinidine, pimozide
• Risk of ergotism with ergot alkaloids
• Significant increase in sirolimus levels

• Heart rhythm disorders
• Liver or kidney problems
• Electrolyte imbalances (correct hypokalemia, hypocalcemia, or hypomagnesemia before treatment)
• Risk of pancreatitis
• Avoid sunlight or prolonged exposure due to risk of photosensitivity
• May cause blurred vision or dizziness—avoid driving or operating heavy machinery
• Monitor closely during pregnancy and lactation
