Adults:

• IV: 6 mg/kg every 12 hours for the first 24 hours, followed by 4 mg/kg every 12 hours.

Esophageal candidiasis (e.g., Candida albicans, Candida glabrata, Candida krusei):

• Oral: 200 mg every 12 hours.
