✔️ Side Effects of Osteo-D

• Common: Anorexia, lassitude, nausea, vomiting, diarrhea, constipation, weight loss, polyuria, sweating, headache, thirst, vertigo.
• Serious: Elevated plasma and urine calcium and phosphate levels.

✔️ Pharmacology

• Mechanism: Active form, Calcitriol, binds to Vitamin D receptors (VDRs) in various tissues.
• Half-Life: Approximately 50 days.
• Absorption and Metabolism: Absorbed in the small intestine, transported to the liver, converted to Calcidiol, and then to Calcitriol in the kidney.
• Storage and Excretion: Stored in fat and muscle tissues; excreted via feces and urine.

✔️ Dosage & Administration of Osteo-D

Injection

• Prevention:
  • Infants: 1/2 ampoule (0.5ml) every 6 months.
  • Children up to 5 years: 1 ampoule (1ml) every 6 months.
  • Adolescents: 1 ampoule every 6 months during winter.
  • Pregnancy: 1/2 ampoule from the 6th/7th month.
  • Elderly: 1/2 ampoule every 3 months.
  • Specific conditions: 1/2 ampoule or 1 ampoule every 3-6 months.
• Deficiency:
  • General: 1 ampoule, repeatable after 1-6 months.

✔️ Interaction

• Increased Hypercalcemia Risk: Thiazide diuretics, calcium, or phosphate.
• Increased Vitamin D Requirements: Antiepileptics (e.g., carbamazepine, phenobarbital, phenytoin, primidone).
• Reduced Efficacy: Rifampicin, isoniazid.
• Counteracting Effects: Corticosteroids.
• Cardiac Glycosides: Increased risk when taken with Digoxin.
• Reduced Absorption: Cholestyramine, colestipol, mineral oil, orlistat.
• Ketoconazole: Possible interaction.

✔️ Contraindications

• Hypercalcemia: Contraindicated in all diseases associated with elevated calcium levels.
• Hypersensitivity: Known hypersensitivity to Colecalciferol or its excipients.
• Vitamin D Toxicity: Evidence of toxicity.

✔️ Pregnancy & Lactation

• Safety: Up to 4,000 IU/day is considered safe.
• Recommended Intake: 400 IU/day, higher if deficient.
• Breastfeeding: Excreted in milk; no observed infant overdose.

✔️ Precautions & Warnings

• Monitoring: Plasma-calcium levels in high doses, renal impairment, and pregnancy.
• Drug Interactions: Consult healthcare providers when using Digoxin, Thiazide diuretics, and for patients with liver/kidney disease, hyperthyroidism, lymphoma, tuberculosis, or granulomatous disease.
• Children (<12 years): Safety and efficacy not established.
