✔️ Dosage & Administration

Adult Dose

• Initial dose: Empagliflozin 5 mg + Metformin 500 mg once daily.
• Dose can be titrated gradually based on effectiveness and tolerance.
• Maximum daily dose: Empagliflozin 25 mg + Metformin 2000 mg.
• Should be taken twice daily with meals to minimize gastrointestinal discomfort.

Pediatric Use

• Not recommended for children under 18 years due to a lack of safety and efficacy data.

Renal Impairment

• Contraindicated in patients with eGFR < 45 mL/min/1.73 m².
• Renal function should be assessed prior to initiation and monitored regularly.

✔️ Empagliflozin 12.5mg + Metformin Hydrochloride 500mg May interact with:

• Insulin & sulfonylureas – risk of hypoglycemia
• Thiazolidinediones may worsen heart failure
• Beta-blockers may mask hypoglycemia symptoms
• ACE inhibitors / ARBs/diuretics – may cause hypotension or dehydration
• Steroids/thyroid hormones may alter blood sugar control
• Alpha-glucosidase inhibitors may reduce glucose-lowering efficacy

✔️ Contraindications

• Type 1 diabetes mellitus
• Diabetic ketoacidosis
• Severe renal impairment (eGFR <30 mL/min/1.73 m²)
• Severe hepatic impairment
• History of lactic acidosis
• Hypersensitivity to any of the components
• Heart failure, dehydration, active urinary tract infection
• Post-operative or trauma patients
• Elderly patients with multiple comorbidities

✔️ Precautions

• Monitor renal function and hydration status.
• Caution in patients with bladder infections, hypotension, or a history of genital infections.
• Avoid alcohol to reduce the risk of lactic acidosis.
• Increased risk of ketoacidosis during stress, illness, or fasting.
