✅ Description:
Empadus M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when dual therapy with empagliflozin and metformin is appropriate. It is also indicated for reducing the risk of cardiovascular death in adults with type 2 diabetes and established cardiovascular disease.
This fixed-dose combination contains:
- Empagliflozin, a selective SGLT2 inhibitor that reduces renal glucose reabsorption and increases urinary glucose excretion.
- Metformin, a biguanide that reduces hepatic glucose production and enhances peripheral insulin sensitivity.
Together, they synergistically lower blood glucose levels and improve insulin action.
If a dose is missed, take it as soon as remembered. If it’s close to the next dose, skip the missed dose. Do not double the dose.
Caution is advised in:
- Elderly patients
- Patients at risk of dehydration
- History of genital infections
- Those undergoing surgery or radiologic procedures involving iodinated contrast agents
- Renal and hepatic function should be assessed before and during treatment

✔️ Dosage & Administration of Empadus M
- Initial Dose: Typically starts with empagliflozin 5 mg + metformin 500 mg once daily with meals.
- Maintenance Dose: May be gradually increased based on clinical response and tolerability to a maximum of empagliflozin 25 mg and metformin 2000 mg per day in divided doses.
- Pediatrics: Safety and efficacy not established in patients under 18 years.
- Pregnancy: Not recommended during pregnancy due to potential fetal risk.
- Lactation: Both components are excreted in breast milk. Use only if clearly necessary.
Administration: Should be taken with food to minimize gastrointestinal side effects.

- Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain)

- Weight loss
- Diabetic ketoacidosis (DKA)
- Lactic acidosis
- Hypoglycemia (especially with insulin/sulfonylureas)

- Hypotension

✔️ Empadus M is contraindicated in:
- Type 1 diabetes or diabetic ketoacidosis (DKA)
- Severe renal impairment (eGFR < 30 mL/min/1.73 m²)
- Severe hepatic dysfunction
- Heart failure
- Active urinary tract infections
- Recent surgery or major trauma
- Known hypersensitivity to empagliflozin, metformin, or any component of the formulation
- History of lactic acidosis

- Increased risk of hypoglycemia with insulin, sulfonylureas, and meglitinides.
- May increase dehydration risk with diuretics.
- Risk of lactic acidosis with alcohol, contrast media, or nephrotoxic agents.
- Beta-blockers may mask symptoms of hypoglycemia.
- ACE inhibitors and ARBs may enhance the risk of hypotension.
- Steroids and thyroid hormones may reduce glycemic control.
