- TCL-R 40 contains atorvastatin, a medicine belonging to the statin group. It is used to lower cholesterol levels and reduce the risk of cardiovascular complications, including heart attacks and strokes. High cholesterol can lead to fatty deposits building up inside blood vessels, narrowing them and restricting blood flow.
- TCL-R 40 is generally suitable for long-term treatment when prescribed and monitored by a doctor. It can be taken with or without food at any time of the day, preferably at the same time each day. Since high cholesterol often causes no noticeable symptoms, regular cholesterol testing and continued treatment are important. Stopping the medicine without medical advice may cause cholesterol levels to rise again.
- Treatment should be combined with a healthy diet, regular exercise, smoking cessation, limited alcohol consumption, and weight management. Avoid excessive consumption of high-fat foods.
- Common side effects include headache, constipation, muscle pain, nausea, indigestion, diarrhea, and nasopharyngitis. These effects are usually mild, but consult a doctor if they persist. Seek prompt medical advice for unexplained muscle pain or weakness, yellowing of the skin or eyes, or unusually dark urine.
TCL-R 40 should not be used during pregnancy or breastfeeding and is contraindicated in active liver disease. Patients with diabetes should monitor their blood glucose levels, as atorvastatin may increase blood sugar in some individuals. Liver function may be assessed before treatment and monitored during therapy.

- Colchicine: May increase the risk of muscle-related adverse effects.
- CYP3A4 inducers: Medicines such as rifampicin and efavirenz may reduce atorvastatin exposure.
- Digoxin: Atorvastatin may increase digoxin concentrations; monitoring may be required.
- Oral contraceptives: Atorvastatin may increase exposure to norethindrone and ethinyl estradiol.

- Continue taking TCL-R 40 regularly, even if you feel well.
- Follow a cholesterol-lowering diet and maintain regular physical activity.
- Avoid excessive grapefruit juice consumption because it may increase atorvastatin exposure.
- Inform your doctor if you experience unexplained muscle pain, weakness, or unusual fatigue.
- Report signs of liver problems, such as dark urine, abdominal pain, or yellowing of the skin or eyes.
- Tell your doctor if you have diabetes, kidney disease, liver disease, hypothyroidism, or a history of muscle disorders.
- Inform your doctor about all medicines you take, particularly certain antibiotics, antifungals, fibrates, ciclosporin, and antiviral medicines.
- Do not take TCL-R 40 if you are pregnant, planning a pregnancy, or breastfeeding.
✔️ TCL-R 40 is contraindicated in patients with:
- Hypersensitivity to atorvastatin or any ingredient in the formulation.
- Active liver disease or unexplained persistent elevations of liver transaminases.
- Pregnancy or breastfeeding.
✔️ Dosage | TCL-R 20
- Adults: The usual starting dose is 10–20 mg once daily. A starting dose of 40 mg may be prescribed for patients requiring a substantial reduction in LDL cholesterol. The maximum recommended dose is 80 mg daily.
- Children aged 10–17 years: For heterozygous familial hypercholesterolemia, treatment generally starts at 10 mg once daily and may be increased to a maximum of 20 mg daily according to the applicable pediatric guideline.
- Dose adjustments are generally made at intervals of four weeks or longer, based on the patient's response and treatment goals.
- Elderly patients generally do not require routine dose adjustment.
