✔️ Dosage & Administration

• Frusemide 20 mg + Spironolactone 50 mg: 1 to 4 tablets daily (20–80 mg Frusemide and 50–200 mg Spironolactone), based on patient response.
• Frusemide 40 mg + Spironolactone 50 mg: 1 to 2 tablets daily for those already stabilized on higher doses (provides 40–80 mg Frusemide and 50–100 mg Spironolactone).
• Pediatric use: Not recommended.
• Elderly patients: Elimination may be slower; dose adjustment may be necessary.

✔️ Drug Interactions

• Combined use with ACE inhibitors or potassium supplements may lead to hyperkalemia.
• Spironolactone can raise serum levels of digoxin, increasing the risk of toxicity.
• Corticosteroids may worsen hypokalemia when used with Spironolactone.
• NSAIDs (e.g., Indomethacin) may reduce Frusemide's diuretic and antihypertensive effects.
• Aminoglycosides may have increased ototoxicity when taken with Frusemide.
• Sucralfate can interfere with Frusemide’s absorption and efficacy.

✔️ This combination is contraindicated in patients with:

• Anuria
• Acute or severe kidney impairment (creatinine clearance <30 ml/min)
• Elevated potassium levels (hyperkalemia)
• Addison’s disease
• Allergy to Frusemide, Spironolactone, or sulfonamides

✔️ Adverse Effects

• Spironolactone may cause: headache, drowsiness, abdominal cramps, diarrhea, confusion, skin rash, and hormonal disturbances (e.g., gynecomastia, voice changes, menstrual irregularities, impotence).
• Electrolyte imbalances: hyperkalemia, hyponatremia, and mild acidosis.
• Frusemide may cause: dehydration, low blood volume, circulatory collapse, and in severe cases, cardiac arrhythmias due to potassium or magnesium depletion.

✔️ Use in Pregnancy and Lactation

Pregnancy: Spironolactone and its metabolites can cross the placenta. Furosemide has shown teratogenic potential in animal studies. Use is only advised when benefits outweigh potential risks, and effective contraception should be ensured in women of childbearing age.

Breastfeeding: Both Spironolactone and Furosemide pass into breast milk. Breastfeeding should be discontinued, or an alternative feeding method should be considered if treatment is essential.

✔️ Precautions

Monitor electrolytes regularly, especially in patients prone to imbalances.

Use cautiously in individuals with diabetes, prostatic enlargement, low blood pressure, or reduced blood volume (hypovolemia).
