- Not suitable for use in children.
- Spironolactone and Furosemide may be excreted more slowly in the elderly.
✔️ Overdose Effects:
- Symptoms may include drowsiness, mental confusion, dizziness, diarrhea, and vomiting.
- Treatment should focus on fluid replacement and correction of electrolyte imbalance.
✔️ Side Effects:
- Headache, drowsiness, gastrointestinal distress.
- Ataxia, mental confusion, skin rashes.
- Gynecomastia, breast enlargement, and other endocrine disorders.
- Transient increase in blood-urea-nitrogen concentrations, mild acidosis.
- Hyponatremia, hyperkalemia.
- Excessive diuresis may lead to dehydration and circulatory collapse.
✔️ Therapeutic Class:
- Potassium-sparing diuretics
- Potassium-sparing diuretics & Aldosterone antagonists
✔️ Pharmacology:
- Furosemide inhibits the Na+/K+/2Cl- co-transporter in the ascending Loop of Henle, increasing the excretion of sodium and water.
- Spironolactone inhibits sodium reabsorption at the distal tubule by antagonizing aldosterone, reducing excess potassium loss induced by Furosemide.
✔️ Dosage & Administration:
- Furosemide 20 mg and Spironolactone 50 mg: 1 to 4 tablets daily (20 to 80 mg Furosemide and 50 to 200 mg Spironolactone) according to the patient’s response.
- Furosemide 40 mg and Spironolactone 50 mg: For stabilized patients requiring higher dosage, 1 to 2 tablets daily (40 to 80 mg of Furosemide and 50 to 100 mg of Spironolactone).
✔️ Interactions:
- Increased risk of hyperkalemia when taken with ACE inhibitors or potassium salts.
- Spironolactone may increase levels of cardiac glycosides, leading to digitalis toxicity.
- Corticosteroids may cause hypokalemia when used with Spironolactone.
- Furosemide's effects may be reduced when used with indomethacin and other NSAIDs.
- Simultaneous administration of sucralfate and Furosemide may reduce their effects.
✔️ Contraindications:
- Anuria, acute renal insufficiency, severe impairment of renal function (creatinine clearance <30 ml/min), hyperkalemia, Addison's disease.
- Hypersensitivity to Spironolactone, Furosemide, or sulphonamides.
✔️ Pregnancy & Lactation:
- Spironolactone and Furosemide may cross the placental barrier.
- Furosemide may cause fetal abnormalities.
- Use in pregnancy and lactation should be carefully weighed against potential risks.
✔️ Precautions & Warnings:
- Caution in patients prone to electrolyte deficiency, diabetes, enlarged prostate, hypotension, and hypovolemia.
✔️ Storage Conditions
- Keep below 30°C, away from light & moisture.
- Keep out of the reach of children.
