Dexamethasone is a potent glucocorticoid with anti-inflammatory and anti-allergic properties. It reduces inflammation by inhibiting phospholipase A2 activity, thereby decreasing the production of inflammatory mediators such as prostaglandins. It also reduces the migration of inflammatory cells, including neutrophils, to affected tissues.
Ciprofloxacin is a fluoroquinolone antibiotic with activity against a broad range of Gram-negative and Gram-positive bacteria. It produces a bactericidal effect by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes required for bacterial DNA replication and survival.

Eye:
This combination is indicated for the treatment of steroid-responsive inflammatory eye conditions where bacterial infection is present or there is a significant risk of bacterial infection. It may also be used for post-operative ocular inflammation and other eye inflammations associated with bacterial infection.
Ear:
It is indicated for the treatment of inflammatory ear infections, including otitis externa and acute or chronic otitis media. It may also be used for post-operative inflammation of the ear when prescribed by a healthcare professional.
Instill 1 drop into the affected conjunctival sac every 4–6 hours.
For more severe inflammation, the frequency may be increased to 1 drop every 2 hours during the first 24–48 hours, followed by a reduction in frequency as the condition improves, according to medical advice.
Acute otitis media in children with tympanostomy tubes:
Instill 4 drops into the affected ear twice daily for 7 days.
Acute otitis externa:
Instill 4 drops into the affected ear twice daily for 7 days.
Pediatric use: Safety and effectiveness in children below 1 year of age have not been established.
Shake the bottle well before use. Avoid touching the dropper tip to the eye, ear, skin, or other surfaces to prevent contamination.

Prolonged use of the steroid component may increase intraocular pressure and may contribute to glaucoma, optic nerve damage, posterior subcapsular cataract, and delayed wound healing.
Ear:

Pregnancy: Adequate, well-controlled studies in pregnant women are unavailable. This medicine should be used during pregnancy only when the expected benefits outweigh the potential risks to the fetus and under medical supervision.
Breastfeeding: It is not known whether the active ingredients are excreted into human breast milk. A healthcare professional should assess the benefits and potential risks before use during breastfeeding.
Specific interaction studies with topical ophthalmic ciprofloxacin and dexamethasone have not been established. However, systemic quinolones may interact with certain medicines, including theophylline, caffeine, warfarin and its derivatives, and cyclosporine.
Although systemic exposure from topical eye or ear administration is generally much lower, inform your doctor about all medicines you are taking.
