✅ বিবরণ:

Apulset Oral Soluble Film is a rapidly dissolving oral film designed to be placed on the tongue. It dissolves in approximately 20 seconds and is swallowed with saliva, so water is not required.

The active ingredient, Ondansetron, is a selective 5-HT3 receptor antagonist that helps control nausea and vomiting.

Ondansetron selectively blocks 5-HT3 serotonin receptors involved in the vomiting reflex. Chemotherapy and radiotherapy can stimulate the release of serotonin in the gastrointestinal tract, which activates vagal nerves and triggers nausea and vomiting.

By blocking these receptors in both peripheral and central pathways, ondansetron helps prevent nausea and vomiting caused by chemotherapy and radiotherapy. Its precise mechanism in post-operative nausea and vomiting is not completely understood but may involve similar pathways.

  • Renal impairment: No routine dose adjustment is generally required.
  • Severe hepatic impairment: The total daily dose should generally not exceed 8 mg.
  • Children: Pediatric dosing depends on age, body weight, and the indication. Use should be determined by a healthcare professional.
  • Elderly: Routine dose adjustment is generally not required in patients over 65 years of age, although individual clinical factors should be considered.

✔️ Chemotherapy-Induced Nausea and Vomiting

Highly emetogenic chemotherapy – Adults:
The recommended oral dose is 24 mg as three 8 mg films, administered approximately 30 minutes before chemotherapy.

Moderately emetogenic chemotherapy – Adults and children 12 years and older:

  • One 8 mg film 30 minutes before chemotherapy
  • One further 8 mg dose 8 hours later
  • Then 8 mg twice daily every 12 hours for 1–2 days after chemotherapy

Children 4–11 years:

  • One 4 mg film 30 minutes before chemotherapy
  • Additional 4 mg doses at 4 and 8 hours after the first dose
  • Then 4 mg three times daily every 8 hours for 1–2 days after chemotherapy
✔️ Side Effects:
  • Headache
  • Constipation
  • Diarrhea
  • Flushing or feeling of warmth
  • Hiccups
  • Mild elevations in liver enzymes
  • Rash

Rare but serious reactions may include:

  • Hypersensitivity or anaphylaxis
  • Bronchospasm
  • Shortness of breath
  • Tachycardia
  • Chest pain
  • Electrolyte abnormalities
✔️ Interaction

Ondansetron is metabolized by hepatic cytochrome P450 enzymes. Medicines that induce or inhibit these enzymes may alter ondansetron exposure. Ondansetron itself does not appear to significantly induce or inhibit the major hepatic drug-metabolizing enzyme systems.

Clinically important interactions may occur with medicines that affect QT interval or cardiac rhythm. Inform your doctor about all medicines you take before starting ondansetron.

✔️ Radiotherapy-Induced Nausea and Vomiting

Adults: One 8 mg film three times daily, as prescribed.

For specific radiotherapy schedules, the dose may be taken 1–2 hours before treatment, with subsequent doses at appropriate intervals according to the prescribed regimen.

✔️ Post-Operative Nausea and Vomiting

Adults: 16 mg, given as two 8 mg films approximately 1 hour before induction of anesthesia.

✔️ Apulset is contraindicated in patients with:
  • Known hypersensitivity to ondansetron or any of its ingredients
  • Concomitant use of apomorphine
✔️ Pregnancy & Lactation

Pregnancy: Adequate and well-controlled studies in pregnant women are limited. Although animal studies have not demonstrated evidence of fetal harm, ondansetron should be used during pregnancy only when considered clinically necessary by a healthcare professional.

Breastfeeding: Ondansetron can pass into breast milk in animal studies. A doctor should assess the potential benefits and risks before use during breastfeeding.

Fertility: Available preclinical data have not demonstrated an adverse effect on fertility.

✔️ Safety Information:
  • Hypersensitivity reactions may occur, particularly in patients who have previously reacted to other selective 5-HT3 receptor antagonists.
  • Ondansetron does not stimulate gastrointestinal motility and should not be used as a substitute for nasogastric suction when clinically indicated.
  • In patients following abdominal surgery or receiving chemotherapy, ondansetron may mask symptoms associated with progressive ileus or gastric distension.
  • Use cautiously in patients with a history of cardiac rhythm abnormalities or other risk factors for QT prolongation.
  • Inform your doctor if you have liver disease, electrolyte abnormalities, or a history of abnormal heart rhythm.
✔️ Storage:

Store at a temperature not exceeding 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.
