✔️ Safety Information:

• Kidney function: Your doctor will assess eGFR before treatment and monitor it during therapy.
• High potassium: Fincard can raise potassium levels. Do not start treatment if your serum potassium is above the recommended threshold.
• Potassium supplements and salt substitutes: Consult your doctor before using products containing potassium.
• Other medicines: Some medicines can increase potassium levels or raise finerenone exposure, increasing the risk of adverse effects.
• Liver disease: Avoid use in severe hepatic impairment. Additional potassium monitoring may be needed in moderate hepatic impairment.
• Low blood pressure: Monitor for dizziness or fainting, particularly if you also take other blood pressure-lowering medicines.
• Regular testing: Attend scheduled blood tests even if you feel well

✔️ Fincard is contraindicated in patients with:

• Adrenal insufficiency
• Concomitant use of strong CYP3A4 inhibitors
• Known hypersensitivity to finerenone or any component of the product
• Treatment should not be initiated when serum potassium is above 5.0 mEq/L. Severe hepatic impairment is also a reason to avoid its use.

✔️ Dosage and Administration | Fincard 20

The dose is determined by the doctor based on kidney function, serum potassium levels and the patient's overall condition.

Recommended Starting Dose for Adults

• eGFR 60 mL/min/1.73 m² or higher: 20 mg once daily.
• eGFR 25 to below 60 mL/min/1.73 m²: 10 mg once daily.
• eGFR below 25 mL/min/1.73 m²: Initiation is generally not recommended.

The usual target dose is 20 mg once daily, if appropriate based on follow-up test results.

Monitoring and Dose Adjustment

Measure serum potassium and eGFR before starting treatment.

Do not initiate treatment if serum potassium is above 5.0 mEq/L.

Check serum potassium approximately 4 weeks after starting treatment and after dose adjustments.

Continue periodic monitoring during treatment. More frequent testing may be required in patients at increased risk of hyperkalemia.

✔️ How to Take Fincard

• Take once daily as prescribed, with or without food.
• Swallow the tablet as directed. If swallowing is difficult, ask your pharmacist or doctor whether crushing the specific product is appropriate.
• Do not change the dose without medical advice.

✔️ Use in Special Populations

Children and Adolescents

Safety and effectiveness have not been established in patients under 18 years of age.

Older Adults

No routine dose adjustment is generally required solely because of age. Kidney function and potassium levels should still be monitored.

Liver Impairment

No dose adjustment is generally required for mild or moderate hepatic impairment, although additional potassium monitoring may be appropriate in moderate impairment. Avoid use in severe hepatic impairment.

Pregnancy

There are limited human pregnancy data. Animal studies have shown developmental toxicity at exposures higher than those expected at usual human doses. Use only if the treating doctor determines that the potential benefit justifies the potential risk.

Breastfeeding

Breastfeeding is not recommended during treatment and for 1 day after the last dose, according to the supplied prescribing information. Discuss appropriate treatment and infant-feeding options with your healthcare professional.

✔️ Interaction

Tell your doctor about all medicines, supplements, and herbal products you use. Important interactions include:

• Strong CYP3A4 inhibitors: These can substantially increase finerenone levels and are contraindicated with finerenone.
• Moderate or weak CYP3A4 inhibitors: These may increase finerenone exposure and require closer potassium monitoring and possible dose adjustment.
• Strong or moderate CYP3A4 inducers: These may reduce finerenone levels and effectiveness; concomitant use should be avoided.
• Potassium-raising medicines: Certain medicines used for blood pressure or heart conditions may increase the risk of hyperkalemia.
• Grapefruit and grapefruit juice: Avoid these during treatment because they may affect finerenone exposure.

Do not start or stop medicines without consulting your healthcare professional.

✔️ Missed Dose & Overdose

• Take the missed dose as soon as you remember on the same day. If you remember on the following day, skip the missed dose and take the next dose at the usual time. Do not take two doses together.
• Overdose may cause hyperkalemia or other adverse effects. If an overdose is suspected, seek urgent medical advice. Treatment may need to be interrupted under medical supervision. Finerenone is unlikely to be efficiently removed by haemodialysis because it is highly protein-bound.
