Recombinant follicle-stimulating hormone (rFSH) is a glycoprotein hormone produced using recombinant DNA technology. It consists of two distinct subunits, alpha and beta, which have structures comparable to those of naturally occurring human FSH. The hormone is manufactured using genetically modified Chinese hamster ovary (CHO) cells and purified to achieve consistent quality and biological activity.
Its activity is standardized against the World Health Organization's international reference standard for recombinant human FSH. Unlike some gonadotropin preparations, rFSH does not possess luteinizing hormone (LH) activity.

In Women:
Ovulation Induction: rFSH is used with human chorionic gonadotropin (hCG) to stimulate ovulation in women who have previously undergone pituitary suppression.
Assisted Reproductive Technology (ART): It stimulates the development of multiple ovarian follicles during controlled ovarian stimulation in women undergoing ART procedures, including IVF, following pituitary suppression.
Infertility Associated with PCOS: rFSH may be used to stimulate follicular development in women with polycystic ovary syndrome (PCOS) who experience infertility.
In Men:
- Male Infertility: In combination with hCG, rFSH is used to stimulate sperm production in men with hypogonadotropic hypogonadism and impaired spermatogenesis.

The severity depends on the dose and individual response. Suspected overdose or symptoms of ovarian hyperstimulation require prompt medical assessment.
rFSH is generally administered by slow subcutaneous injection. Injection sites should be rotated to reduce the risk of local tissue changes, and any unused solution should be discarded. Self-administration may be considered only after proper training and under medical guidance.
Dosage in Women
The dose varies considerably between individuals and must be adjusted according to ovarian response. Ultrasound examinations and serum estradiol measurements are required to monitor follicular development and minimize the risk of ovarian hyperstimulation. rFSH may be used alone or with a gonadotropin-releasing hormone (GnRH) analogue, depending on the treatment protocol.
Ovulation Induction
A typical starting dose is 75 IU subcutaneously once daily for at least seven days. Depending on follicular growth and estradiol levels, the dose may be increased gradually under medical supervision. Once appropriate follicular development is achieved, hCG may be administered to trigger final oocyte maturation and ovulation.
Assisted Reproductive Technology (ART)
The usual starting dose may range from 150 to 300 IU daily for at least the first four days. Subsequent doses are adjusted according to ultrasound findings and estradiol levels. When follicular development is adequate, hCG may be given to trigger final oocyte maturation. Egg retrieval is generally performed 34–36 hours after the hCG injection.
PCOS-Related Infertility
Treatment commonly begins with a low daily dose, often 75 IU, administered subcutaneously. The dose and duration are adjusted according to the individual's ovarian response, assessed through hormone measurements and ultrasound monitoring. Increases must be made cautiously to reduce the risk of ovarian hyperstimulation.
Dosage in Men
For men with hypogonadotropic hypogonadism, rFSH is used with hCG to stimulate sperm production. hCG is generally administered first to achieve appropriate testosterone levels, after which rFSH may be added. Treatment doses and follow-up should be determined by a specialist, with semen analysis commonly performed after four to six months to assess response.
The doses above are general reference information, not a self-treatment schedule. rFSH treatment requires specialist supervision and monitoring.
For a 75 IU vial, the supplied instructions may specify dissolving the powder in 1 mL of water for injection. Gently swirl the vial until the solution becomes clear; do not shake vigorously. Do not use the solution if it contains particles or appears cloudy.
If multiple vials are required, reconstitution and transfer must follow the exact product instructions and be performed using proper sterile technique by a trained healthcare professional. The appropriate diluent and preparation method may differ by brand.
rFSH must not be used during pregnancy or breastfeeding. Treatment should be initiated only after pregnancy has been excluded and the prescribing specialist has assessed the patient's suitability.
In women without primary ovarian failure, rFSH treatment is commonly administered for approximately 7–12 days to stimulate ovarian follicle growth and maturation. Once adequate follicular development is confirmed, hCG may be administered to trigger ovulation or final egg maturation, according to the treatment plan.
rFSH may cause ovarian overstimulation in women. Possible symptoms include pelvic or abdominal pain, bloating, nausea, vomiting, and sudden weight gain. Severe cases may lead to breathing difficulties, reduced urine output, fluid accumulation in the lungs, or blood clots.
Other serious warning signs include severe pelvic pain, chest pain, or significant breathing problems. Treatment may also increase the chance of multiple pregnancy, including twins or higher-order pregnancies.
Store in a refrigerator at 2°C–8°C and do not freeze. Keep out of reach of children. Always follow the specific product packaging for storage requirements before and after reconstitution.
